synopsis/solicitation for commercial items prepared in accordance with
the format in FAR Subpart 12.6, the test program in FAR Subpart 13.6,
and FAR Part 15, as supplemented with additional information included
in this notice. This announcement constitutes the only solicitation;
proposals are being requested and a written solicitation will not be
issued. 17(ii). Solicitation Number 7T-98-2002 Request for Proposal.
17(iii). This solicitation document and incorporated provisions and
clauses are those in effect through the issuance of Federal Acquisition
Circular 97-2. 17(iv). This procurement is set aside for small business
participation under SIC 3577, Computer Peripheral Equipment, Not
Elsewhere Classified. The small business size standard is 1,000
employees. 17(v). Requirements listed below. 17(vi). Description: The
purpose of this Request for Proposal is to acquire the following
computer peripheral equipment: Howtek ScanMaster 2500 scanner with
transparency option (or equal), to include 90 days parts and labor,
depot warranty; Howtek Polaris RGB software (or equal) for PC platform;
and one year extended warranty for equipment specified above. Pricing
must also include FOB Destination shipment, installation and training.
Shipping may be included as a separate line item. 17(vii). Date of
delivery is no later than 30 days after receipt of order. Place of
Delivery and Acceptance is: Wilford Hall Medical Center, 2200 Bergquist
Drive., Suite 1, Lackland AFB, TX. FOB Destination. 17(viii).
Solicitation provision at FAR 52.212-1, Instructions to Offerors --
Commercial Items (JUN 1997) is hereby incorporated by reference.
17(ix). The Government will award a contract resulting from this
solicitation to the responsible offeror whose offer conforming to the
solicitation will be most advantageous, representing the best value to
the Government price and other factors considered. The following
factors shall be used to evaluate offers: Evaluation will be based on
(1) Ability to meet the requirements listed under item 17(vi) within
the time frame specified and (2) Price. Factors are listed in their
relative order of importance. 17(x). Offerors are reminded to include
a completed copy of the provision at 52.212-3, Offeror Representations
and Certifications -- Commercial Items (JAN 1997), with offer. 17(xi).
Clause 52.212-4, Contract Terms and Conditions -- Commercial Items (MAY
1997), is hereby incorporated by reference. 17(xii). Clause 52.212-5,
