synopsis/solicitation for commercial items prepared in accordance with
the format in FAR Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only soliciation; proposals are being requested and a written
solicitation will not be issued. This solicitation F4561398Q0096 is
issued as a request for quotation (RFQ). The solicitation document and
incorporated provisions and clauses are those in effect through the
Federal Acquisition Circular 97-02. SIC Code is 7352, Small Business
Size Standard is $3.5 Million. This requirement is for a monthly lease
of the "Baker" Automated Prescription System for 92nd Medical Group
(Bldg #2460 & 9000) Fairchild AFB, WA. Bldg # 2460 (Satellite Pharmacy)
Bldg. # 9000 (Hospital Pharmacy) (117 ea) Drug Cell w/LED Readout (11
ea) Super Cell w/LED Readout (12 ea) Super Cell w/LED Readout (165 ea)

Counter The above requirement shall include: A. Cabinetry B. Delivery
and Installation (if applicable) C. In-serice training a tthe time of
delivery (if applicable) D. Mechanical locking E. 800 Number (or equal)
to report defective equipment or request updates F. Updates ( at no
additional cost) Once the contractor has received notification from
personnel authorized to place calls directly to the contractor, the
contractor shall ship replacement(s) for defective drug cells. If
determined an emergency by MTF personnel, the contractor shall ship via
air transportation, i.e., UPS, Federal Express, or equal, within 24
hours after notification (excluding weekends and holidays). Defective
cells determined by MTF personnel to be of routine nature may be
shipped by ground transportation. The MTF will only request shipment
via air for replacement of defective cell(s) in extreme situations. The
MTF will ship at the governments expense any defective cell (s) within
2 working days. The contractor shall provide all parts, tools,
materials, labor, and transportation necessary to provide this
requirement and will bill monthly in arrears. Performance Period: 5 Dec
97 -- 30 Sep 98 FOB destination pricing required with acceptance at the
Hospital & Satellite Pharmacy at Fairchild AFB, WA. The provision at
FAR 52.212-1, Instructions to Offerors-Commercial Items (Oct 1995)
applies to the acquisition. Offerors may submit their offer via mail or

evaluation factor of paragraph (a), in descending order of importance
