W -- RENTAL OF NUMERIC DISPLAY PAGERS SOL RFQ 665-2-98 DUE 112197 POC Chris Crader (818)895-9365 W -- RENTAL OF 290 LONG RANGE NUMERIC DISPLAY PAGERS. This is a combined synopsis/solicitation for commercial items prepared utilizing acquisition procedures for Commercial items in Federal Acquisition Regulation (FAR) Subpart 12.6 as supplemented with additional information in this notice. This announcement constitutes the only solicitation; proposals are being requested, a written solicitation will not be issued. This solicitation 665 RFQ-2-98 is being issued as a Request for quotation (RFQ). This document incorporates provisions and clauses which are in effect through FAC 90-46, March 17,1997. This acquisition is a 100% set-aside for small business. The services to be provided as follows: 1. Rental and service of 290 long-range display pagers; coverage shall be for 24 hour service each day, 7 days per week. Pager range must be a 280 mile radius, which includes all areas of the SO. Calif. Systems of Clinics and VISN 22 (this area includes San Luis Obispo, San Diego, Bakersfield and the Las Vegasarea). The offeror shall furnish five (5) blank pagers for the following locations: SCSC Sepulveda, 16111 Plummer Street, Sepulveda, CA. 91343 and the Los Angeles Outpatient Clinic, 351 East Temple St. Los Angeles, CA. 90012-3328. The offeror shall provide all labor, materials, equipment transporatation, and supervision necessary to provide pager service. Activation of pagers must be available during normal business hours 8:00am-4:30pm, Monday through Friday. Pagers shall have the following capabilities: (a)Contain an on/off switch (b)Numeric display pager must have capability to leave a voice or digital response on the same pager phone number (c)If not manually reset, after a message the pager will automatically reset (d)Time/date stamp record for messages received (e)If pager is turned off the messages and time of day will remain in memory (f)Allow the user to put in a personal (voice) greeting message (g)Capability which allows the user to replay, delete or store/save messages (h)Audible and/or visual alert digital display (will emit a beep when message is received) (i)Silent mode-pager will not give an audible alert (it will only vibrate when message is received. 2. Rental of four (4) 1-800 numeric display pagers which have the range of the continental united States, separate individual 1-800 numbers for each pager (no pin number required). Activation of pagers must be available during normal business hours as listed above. The pagers shall have the following capabilities: (a)Contain on/off switch (b)Numeric display pager must have the capability to leave either a voice or digital response on the same pager phone number (c)If not manually reset, after a messge the pager will automatically reset (d)Time/date stamp record for messages received (e)If the pager is turned off the messges and time of day will remain in memory (f)Allow the user to put in a personal (voice) greeting message (g)Capability which allows the user to replay, delete or store/save messages (h)Audible and/or visual alert digital display (will emit a beep when message is received) (i)Silent mode-pager will not give an audible alert (it will only vibrate when a message is received). 3.Yearly training by the contractor for new staff is required without additional charge. All offerors must furnish a toll free number to be made available to pager users so they can check their messages without charge to the calling party, when party is outside the 818 area code. 4. The solicitation requires option to delete and/or add pagers as required. This solicitation includes basic year with three option years. Offerors responding to this solicitation shall provide quotations for all of the above years. Quotations that do not include prices for all options may be determined non-responsive. Prices shall be per pager per month.
