notice is provided on a sole source basis, with intent to award a firm
fixed price order with UCSD Medical Genetics. This is a combined
synopsis/solicitation for commercial items, prepared in accordance with
the format FAR Subpart 12.6 as supplemented with additional information
included in this notice. This announcement constitutes the only
solicitation; other interested parties may submit propsals to the
address indicated above Attn: Michael Gregory. A written solicitation
will not be issued. The solicitation, number N00259-97-Q-0184, is
issued as a Request for Quotations (RFQ). The solicitation document and
incorporated provisions and clauses are those in effect through FAC
90-46. The standard industrial classification (SIC) code for this
acquisition is 8071. Calls will be placed from 01 Oct 97 through 31 Oct
97 against Blanket Purchase Agreements (BPA's), however, the exact
number of tests cannot be pre-determined. The number of required tests
may fluctuate depending on patient care. The typesof laboratory
services required under this effort will include: amino
karyotype/chromosome amniotic fluid, blood chromosome, bone marrow
chromosome, DNA fragile X, tissue chromosome. Contractor must provide
to the Naval Medical Center, San Diego on site access to consultants
who participate in genetic counselling and specimen collection. In
addition, contractor may be used on a consult basis in those patient
care cases where specialized equipment is needed and will be used on
their premises. Lab testing must be performed Monday through Friday and
the contractor will be required to provide lab results to the Naval
Medical Center, San Diego by means of either facsimile, mail, or
contractor provided printer. This acquisition incorporates the
following FAR clauses: 52.212-1 INSTRUCTIONS TO OFFERORS-COMMERCIAL
ITEMS (Oct 95); 52.212-2 EVALUATION-COMMERCIAL ITEMS (Oct 95) is
incorporated by reference with the following addenda: in paragraph (a)
the government will evaluate proposals based on past performance,
technical capability and cost to the government. All other factors when
combined, make up 50 percent of evaluation factors as compared to
price; 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATION-COMMERCIAL
ITEMS (Oct 95) must be completed and submitted along with the proposal
(a copy can be obtained by contacting Michael Gregory at
619/532-5362); In accordance with FAR clause 52.212-5 CONTRACT TERMS
AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES OR EXECUTIVE
ORDERS-COMMERCIAL ITEMS (Oct 95), the following clauses are hereby
incorporated by reference in paragraph (b): 52.222-26 EQUAL OPPORTUNITY
(E.O. 11246); 52.222-35 AFFIRMATIVE ACTION FOR SPECIAL DISABLED AND
VIETNAM VETERANS (38 U.S.C. 4212); 52.222-36 AFFIRMATIVE ACTION FOR
HANDICAPPED WORKERS (29 U.S.C. 793); and 52.222-37 EMPLOYMENT REPORTS
ON SPECIAL DISABLED VETERANS AND VETERANS OF THE VIETNAM ERA (38 U.S.C.
4212). Funds subject to the enactment of the FY98 DOD appropriation act
or the FY97 continuing resolution authority. All provisions apply of
whichever act becomes available. FAR clause 52.232-18 (Availability of
Funds April 1984) applies. Interested sources should provide pricing
