combined synopsis/solicitation for commercial items prepared in
accordance with the format in FAR Subpart 12.6 as supplemented with
additional information included in this notice. Contractor shall
provide storage, delivery, pick-up and repair of Government Owned
Equipment for the Department of Veterans Affairs Medical Centers, 10000
Brecksville Road, Brecksville, Ohio and 10701 East Boulevard,
Cleveland, Ohio 44106. This announcement constitutes the only
solicitation; proposals are being requested and a written solicitation
will not be issued. The Solicitation Number is 541-37-98 and is issued
as a Request For Quotations (RFQ) under commercial items. This
solicitation document incorporates all applicable provisions and
clauses in effect through Federal Acquisition Circular 90-46. This
procurement is 100% set aside for small business. The applicable SIC
code is 4214 and the size standard is $12.5 million. Quotations must
reach the Contracting Officer no later than $;#0 p.m. local time on
September 15, 1997. The provision at 52.212-1, Instructions to Offerors
-- Commercial Items applies to this acquisition.52.212-2 applies and
quotations will be evaluated on price and past performance. Location
and ability to provide repair within a reasonable amount of time. The
VA shall award to the company offering the best overall value to the
Government. Offerors should include a completed set of Representations
and Certifications -- Commercial Items with their quotation. You may
obtain from the Contracting Officer if needed. 52.212-4, Contract Terms

pick-up of misc. medical equipment such as hospital beds; lifts;

working condition. Parts and extra spare parts and/or replacements will

One-way only from Brecksville Unit and Wade Park Unit approximately 800
miles per year for initial year: October 1, 1997 through September 30,
1998 @$__ __ ____. Option Year 1: October 1, 1998 through September
30, 1999 @$&llmdash;_ __ ____. Option Year 2: October 1, 1999 through
September 30, 2000 @$__ __ ____. Line Item 5: General Labor to move
equipment within VA facility, rate per person, per hour, estimated @ 50
hours per year for initial year: October 1, 1997 through September 30,
1998 @$__ __ __ ___.Option Year 1: October 1, 1998 through September
30, 1999 @$__ __ __ __ ___. Option Year 2: October 1, 1999 through
September 30, 2000 @$__ __ __ ____.Contractor shall check (externally)
electrical outlets in client's home and advise the Chief of
Prosthetics or his designee. Contractor shall advise client or
caregiver of their responsibility to bring the electrical outlets up to
current safety standards. Contractor shall instruct client or caregiver
the proper use of equipment, Contractor shall instruct client or
caregiver whom to call 24 hrous hours a day, or seven days a week in
case of equipment breakdown. Contractor and staff will be trained by
Chief of Prosthetics or his designee on VAMC home health care
procedures including Infection Control Procedures.Contractor will wash
and disinfect all equipment for reissuance with a LPH Solution or
equivalent. Contractor must be JCAHO accredited at time of award and
must be credit card adaptable. Listing of government equipment will
consist of but not limited to: Electric/manual beds; trapeze units;
hoyer lifts; electric/manual wheelchairs and overhead tables.
Contractor shall pick up equipment within two (2) days after
notification, equipment shall be cleaned, inspected and placed in first
class working condition prior to placement in storage and delivery to
clients shall be accomplished within 48 hours after notification.
Contractor shall guarantee repairs and workmanship for a period of 90
days. Geographic areas shall include Ohio; Pennsylvania, West Virginia,
Indiana, and Michigan. Credentials of contractor staff will be required
