This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared
in accordance with the information in FAR subpart 12.6 as supplemented
with the additional information included in this notice. This
announcement constitutes the only solicitation. PROPOSALS ARE BEING
REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation
number is DADA10-97-R-0059 and is issued as a Request for Proposal
(RFP). The solicitation document and incorporated provisions and
clauses are those in effect through Federal Acquisition Circular 90-46.
This procurement is 100% small business set-aside. The standard
industrial classification code is 7338. The small business size
standard for this procurement is $5M. The Government seeks to award a
firm-fixed price indefinite delivery/indefinite quantity contract. The
contract period will be from date of award through 30 Sept. 2000.
Contract line item: CLIN 0001 -- Estimate of 200,000 each employee
medical records (EMR). The Division of Federal Occupational Health
(FOH) provides a variety of health services to federal employees
primarily through Occupational Health Centers (OHCs) and
Wellness/Fitness Centers located throughout the United States, which is
recorded in an individualized medical record. The medical records are
held in the OHCs for six years from the date of the employee's last
visit in accordance with federal regulations. The Government has an
initial estimated 170,200 records, and an estimated on-going
requirement of 10,000 records per year. The purpose of this contract is
for the contractor to receive and screen all records for
appropriateness for long term storage, prepare records for destruction
if they are not required to be stored (according to applicable
regulations) and destroy, prepare remaining records for long term
storage, and provide originating OHCs with documentation of final
disposition of all records. The applicable federal regulations specify
requirements for handling, storage and destruction of medical records
and have been established by the Office of Personnel Management (OPM),
Occupational Safety and Health Administration (OSHA), National Archives
and Records Administration (NARA). The references are NARA General
Records Schedule 1, 5CFR 293.501 Subpart E, and 29 CFR 1910.20 Records
Access Rule. The contractor will be required to furnish a facility
large enough to house records while they are being reviewed for
retention/destruction. The facility must be adequate to maintain
confidentiality of these medical records. The contractor shall supply
all equipment and materials (such as labels, boxes, office supplies,
and etc.) to perform this service. The contractor will prepare records
to be retained in accordance with National Personnel Records Center
instructions. All records are to be contained in a SF66D folder (which
will be supplied by the government) and labeled with the employee's
last name, first name, middle initial, social security number, and date
of birth. If requested the contractor must be able to retrieve records
within 15days, such requests will be extremely rare. Long term
retention will be based on such documentation as pre-placement
physicals, medical surveillance exams, vaccinations other than
influenza and pneumonia, fitness for duty examinations, driver
physicals, medical opinions and progress notes, health risk appraisals,
and Tb skin testing after 1994. Records for long term storage will be
shipped by contractor to National Personnel Records Center in St.
Louis, MO. The reporting process requires a progress report describing
the volume of work, number of records reviewed, number of records
destroyed and the number of records retained. A monthly report will be
sent to the originating Occupational Health Center listing the last
entry, employee name, social security number, date of birth, date of
destruction or the date it was sent to St. Louis. To be considered for
award offerors must submit as part of their proposals information that
addresses the following evaluation factors. 1.) PAST PERFORMANCE:
Submit a list of past contracts (during the last three years) that are
similar in nature to the services required in this solicitation. 2.)
TECHNICAL CAPABILITY: Submit a narrative describing your company's
capability and explain how your company will perform these services.
3.) PRICE: Offeror shall submit a unit price for CLIN 0001(price per
each employee medical record). FAR provision 52.212.1, Instructions to
offerors-Commercial is hereby included by reference with the following
addendum: Paragraph (c) Period for acceptance of offers: the offeror
agrees to hold prices in their proposal firm for 90 calendar days from
the date specified for receipt of offers. FAR provision 52.212.2,
Evaluation Commercial items is incorporated by reference with the
following addendum: Paragraph (a) The following factors shall be used
to evaluate offers: Past Performance, Technical Capability and Price.
Past Performance and Technical Capability are of equal importance and
both combined are significantly more important than price. FAR
Provision 52.212.3, Offeror Representations and
Certifications-Commercial Items must be completed and submitted with
proposal, FAR Clause 52.212-4, Contract Terms and Conditions-Commercial
Items applies to this solicitation. FAR Clause 52.212-5, Contract Terms
and Conditions Required to Implement Statutes or Executive
Orders-Commercial Items applies to this acquisition with the following
addenda: Paragraph (b) -- the following FAR clauses are incorporated
by reference: 52,203-6, 52.203-10, 52.219-8, 52.219-14, 52.222-26,
52.222-35, 52.222-36, and 52.222-37. Paragraph (c) -- the following
clauses are incorporated by reference: 52.222-41, 52.222-42, and
52.222-43. The Department of Labor Wage Determination applicable to the
successful awardee's geographical location will be incorporated in the
