R -- DATA COLLECTION FROM ALZHEIMER'S DEMONSTRATION PROGRAM GRANTEES SOL HRSA-97-BPHC-A219570 DUE 082897 POC Aileen Broider-301-443-5231 The Health Resources and Services Administration intends to negotiate on a SOLE SOURCE basis with the University of Kansas, Gerontology Center, 4009 Dole Building, Lawrence, KS, 66045. This procurement shall be processed in accordance with the Simplified Acquisition Procedures in Part 13 of the Federal Acquisition Regulations. The purpose of this procurement is to support the continuation of data collection of all program data from the 15 Alzheimer's Demonstration Grant Program State grantees; revise elements of data collection instruments to include new elements of the program; and provide consultation to the project. The contractor shall: contact grantees on a regular basis to arrange transfer of data and review data forms for completeness and accuracy; determine all revisions in the data collection elements; submit regular reports on data collection decisions made with grantees; and, attend and provide prepared evaluation presentation at meetings. The University of Kansas, Gerontology Center is deemed the only responsible source capable of providing the required work and services. The period of performance is eight months.
