Wolfe, 304/285-5883 THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This notice, issued as Amendment 001 to Solicitation No. RQ97-026, answers questions by providing additional information and/or acceptable models for the items listed in the initial CBD Announcement appearing August 5, 1997. The solicitation is changed to substitute the following descriptions in lieu of those stated in the initial notice. (Any brand micropipeter is acceptable as long as the Volumes specified for Items 002 through 006 are identical to those stated in this amendment.): (001) Eppendorf Pipetter Repeater (#22-26-000-6) or equal -- 7 each (Lists part number);(002) Micropipetors Volume .5-10 -- 8 each (Amendment changes volume); (003) Micropipetors Volume .2-20 -- 8 each (Amendment changes volume); (004) Micropipetors Volume 10-100 -- 5 each (no change); (005) Micropipetors Volume 50-250 -- 5 each (no change); (006) Micropipetors, Volume 100-1000 -- 5 each ( no change); (007) Eppendorf Microcentifuge Model 5415C or equal -- 5 each (no change); (008) Carousel Stand for Micropipetors -- 17 each (no preference). The Government will evaluate based on F.O.B. Destination (Morgantown, West Virginia) and the requirements at FAR 52.214.21 "Descriptive Literature" apply to referenced brand name products. Information provided must demonstrate products offered are equal when applicable or your offer may be rejected. The Government desires delivery of all line items within 2 weeks after award of order. Offers shall state a delivery schedule. All FAR provisions and clauses stated in the initial notice apply without change. Quotes remain due August 20, 1997, 2:00 p.m. local prevailing time. Submit the following to DHHS, PHS, CDC, NIOSH, ALOSH, Attn: Brenda Goodwin, 1095 Willowdale Road, Morgantown, WV 26505: (1) original and two copies of a completed SF 1449, signed by an individual authorized to bind the organization; (2) Schedule of Offered Supplies/Services containing pricing of individual items (001-008), delivery schedule and published price lists (if applicable) If GSA pricing is available, the contract number should be included in your offer.; (3) completed Representations and Certifications -- Commercial Items, FAR 52.212-3; (4) descriptive literature, tables, etc. which demonstrate compliance of equal products for evaluation purposes; and (5) acknowledgment of solicitation amendments, if any. Offers that fail to furnish the required information, or reject the terms and conditions of the solicitation may be excluded from consideration.
