Q -- SCARCE MEDICAL SPECIALIST FOR RETINAL ANGIOGRAPHY SOL RFP691-80-97 DUE 082297 POC JEWEL JACKSON, Contract Specialist This is a combined synopsis/solication for commerical items prepared in accordance with FAR Subpart 12.6 and supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Request for Proposal (RFP) are being requested and a written solicitation will not be issued. Solicitation document an incorporated provisions and clauses are those in effect through Federal Acquisition Circular 46. This solicitation shall be 100% small business set aside. The SIC code 8099 and the small business size is $5M. Contractor shall provide the services listed in this solicitation, no partial bids will be accepted. The quantity on items required are estimated as follows: 300 each Fluorescein Angiography (color fundus photography included) RVS Code 92235; 300 each Color fundus photgraphy RVS Code 92250; 30 each Color external photography (i.e. macro and slit lamp photography) RVS Code 92285. All equipment will be furnished by the DVA Medical Center; however in the eventof equipment failure, the offeror should include pricing for the following: 5 days for the Power Supply and 25 days (5 weeks) for the Camera Body. Successful bidders shall be required to provide the above listed services and equipment.
