Lopes The Veterans Affairs Medical Center, 5901 East 7th Street, Long
Beach, CA intends to award a contract for an Orthopedic Scanner. This
is a Combined Synopsis/Solicitation for commercial items prepared in
accordance with subparts 12.6 as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. This solicitation is being issued as
a request for proposal. The solicitation and incorporated provisions
are those in effect through Federal Acquisition Circular 90-46. Items
required should include (1 Ea) Orthotic Scanner, (1 Ea) Orthotic
Scanner Wall Mount. (1 Ea) Complete Software package for scanning,
modeling and milling -- to include patient data Record System, Software
to be compatible with an IBM compatible 486, 33 DX MHZ or higher, (1
Ea) Software to Interface a Digitizer, (1 JB) On-site installation, (1
JB) Three Days of On-site Training, (2 Ea) Operations and Training
Manuals, (2 Ea) Service Data Manuals and (2 Ea) Parts Manuals. The
Orthotic Scanner shall be designed for measuring the leg, arm, torso
and head of the patient for production of Orthoses, helmets, etc. It
shall also be able to measure the foot for production of insoles as
well as orthopedic shoes. Scanner must be able to be used in a vertical
or horizontal position (when mounted to the wall using the Wall Stand).
Scanner shall have two measuring ranges to continuously vary its
measuring position relative to the base plate. The software for
modeling shall be user friendly, controlled by a mouse and pull down
menus. Software must include a patient data record system according to
ISO Standard, a high resolution graphics system allowing objects to be
viewed as sections and solid 3-D models which can be rotated and moved
in real time on screen. The system must be compatible with our
existing CAD/CAM Carver which manufactures Orthotic devices and our
existing Vacuformer. The following incorporated clauses and provisions
are applicable to this acquisition: 52.212-1 Instructions of Offerors
-- Commercial Items. Offerors are to include a completed copy of the
FAR Provisions 52.212-3 with their offer also included; 52.212-4,
Contract Terms and Conditions-Commercial Items 52.212-5, Contract Terms
and Conditions required to implement statues or executive orders
Commercial Items; 52.233-3, Protest After Award; 52.222-26 Equal
Opportunity; 52,232-33 Mandatory Information for Electronic Funds
Transfer, and VAAR Provision 852.219-70. VAAR Clauses 852.237-7 and
852.271-70 apply to this acquisition. Contractors who wish to be
considered must submit their proposals on Company letterhead along with
commercial literature and brochures of the items being offered that
meet Government requirements. Any interested parties must submit their
proposals no later than August 18, 1997 at 4:00 PM Pacific Standard
Time. No Fax or telephonic proposals will be accepted. The Government
may evaluate offerors and award a contract without discussion with
offerors. Therefore, the proposal should contain the offeror's best
