This announcement constitutes the only solicitation; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation number is DADA10-97-Q-0143 and is issued as a Request For Quotation (RFQ). This solicitation documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 90-46. This acquisition is unrestricted, Standard Industrial Classification (SIC) is 3845 and Small Business size standard is 500 employees. The provision at FAR 52.212-1, Instructions to Offerors-Commercial, with addenda: paragraphs (h) and (i) of this provision do not apply and change paragraph (c) from 30 calendar days, to 60 calendar days, applies to this acquisition. The U.S. Army Medical Command, Central Contracting Office (CCO), intends to award a firm fixed price Purchase Order for (1 ea) Eclipse TMR 2100 Holmium Laser System; (2 ea) Crystal Point Fibers; (4 ea) Crystal Flex Fibers; and (2 ea) Sologrip Handpiece; for Transmyocardial Revascularization (TMR) at Brooke Army Medical Center (BAMC), Fort Sam Houston, Texas. Specifications are as follows: LASER SYSTEM: Mobile, Solid State, Self-Contained, Internally Cooled, Cold Pulsed and require no external running water; wave length of 2.1 microns in the mid-infrared spectrum; High peak pulse energy of approx 3.2 joules per pulse; 250 micro second pulse width with a pulse rate of 5 pulses per second; Range of 16 watts minimum and 20 watts maximum; 6,7, or 8 watts delivered to the tissue; 15 second warm up cycle; Compact unit with approx weight between 400-500 lbs; 220 VAC, 50/60Hz, 20 amps; Hubbel 250V 20A electrical plug; Overall dimensions in inches approx 46Lx22Wx42H; Included shall be Two (2) pairs of safety glasses, two (2) pairs of safety goggles and two (2) laser warning signs placed at each access way to the treatment room. CRYSTAL POINT FIBERS: 1mm rigid fiberoptic with a single-use aluminum handle. CRYSTAL FLEX FIBERS: 1mm flexible multi-fiberoptic with up to a 15mm bend radius capability for use with the J-Grip hand piece. SOLOGRIP HANDPIECE: 15cm working length and a atraumatic distal head and malleable distal shaft. Vendor shall provide: Initial setup including user documentation manuals; Test, adjust and certify proper performance; Safety training; Training for staff personnel to include travel of one (1) surgeon and assistant, one (1) lead operating nurse and assistant, inclusive of all tuition, manuals, transportation, and lodging for all training required at vendors facilities. At a minimum one (1) year warranty on all parts and labor with option to purchase extended service contracts. Delivery shall be within 30 days after receipt of order, FOB Destination in accordance with FAR 52.247-34.
