Q -- OPTOMETRISTS SOL 124-0028 DUE 082797 POC Ronn Lindert, Contracting Officer, 608-584-6119 The Federal Correctional Institution, Oxford, WI is soliciting to establish contracts with two Optometrists to provide specialty medical services in the area of optometry and ocular (eye) needs to inmates incarcerated at said facility. Offerors to provide care, evaluation and treatment of ocular disease, the prescribing of glasses and availability of glasses for purchase if patients so chose. Offerors will provide full optometric care, provide medical administrative staff with prescriptions for eyeglasses and/or written reports of consultations referred by the institution medical staff for each individual inmate seen. Services to be provided in two hour sessions, one session per weekper optometrist, for an estimated 51 sessions per contractor for the contract period of October 1, 1997 through September 30, 1998. Offerors must be a graduate from an approved school of Optometry with current State of Wisconsin licensure and/or certification to be considered for award. Offerors must have liability insurance issued by a responsible carrier in order to be eligible for award. There will be a pre-bid conference/site visit at the Federal Correctional Institution on August 6, 1997. A firm fixed-price contract will be awarded on or about September 3, 1997 to the lowest qualified bidder. This acquisition is 100% set-aside for small business.
