Q -- DENTAL HYGIENIST SOL 124-0029 DUE 082797 POC Ronn Lindert, Contracting Officer, 608-584-6119 The Federal Correctional Institution, Oxford, WI is soliciting to establish a contract with a Dental Hygienist to provide services to inmates incarcerated at said facility. Services to include but are not limited to; providing oral hygiene education, dental prophylaxis, evaluation and assessment of dental pathology, exposing and developing of roentgenograms, removal of soft and hard deposits, scaling, root planning, curettage and assisting the dentist in select periodontal procedures. Contractor to provide two hour sessions, fifteen sessions per week, for an estimated 720 sessions for the contract period of October 1, 1997 through September 30, 1998. Offerors must be a graduate from an approved school of dental hygiene with current State of Wisconsin licensure and/or certification to be considered for award. Offerors must have verification of credentials and proof of liability insurance issued by a responsible carrier in order to be eligible for award. There will be a pre-bid conference/site visit at the Federal Correctional Institution on August 6, 1997. A firm fixed-price contract will be awarded on or about September 3, 1997 to the lowest qualified bidder. This acquisition is 100% set-aside for small business.
