synopsis/solicitation for commercial items prepared in accordance with
the format in FAR Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. The solicitation # is 459-97-0025.
This is a request for quotation, (RFQ). The solicitation document and
incorporated provisions and clauses are in effect through Federal
Acquisition Circular 90-46. (initial year of service), October 1, 1997
thru September 30, 1998 -- DESCRIPTION: Bio-Hazardous waste pick-up
and disposal Quantity: 12, Unit: Month, Price: $__ ____, There are
options for four additional years of service. Offers, Please submit
pricing for optional years as well. Description is same as above, but
for the period October 1, 1998 thru September 30, 1999, October 1, 1999
thru September 30, 2000, October 1, 2000 thru September 30, 2001 and
October 1, 2001 thru September 30, 2002 respectively. REQUIREMENTS:
Contractor will provide all material, labor, vehicles, and supplies to
pick-up, remove and dispose of biohazardous waste from the Department
of Veterans Affairs Center for Aging, 110 Krukowski road, Tripler Army
Medical Center, Honolulu, Hi 96819 for one base year with four
additional option years. The pick-up, removal, transport and disposal
will be in accordance with the State of Hawaii Administrative Rules,
Title 11, Department of Health Chapter 104, Management of Infectious
Waste, current edition. Contractor will provide red, bio-hazard trash
liners and bio-hazard labels for four (4), 12 gallon stainless steel
step on cans (VA provided) located in rooms A109, A126, B123 and C106.
Pick-up of infectious waste will be once a week each Thursday to
include disinfecting of receptacles. Contractor will also provide a
2-gallon sharps container with a wall-mount bracket and lock in the
following rooms: A118 Pharmacy, A133 Physicians Exam Room, A136 Exam
Room, A139 Exam Room, B105 Medication Room, B134 Exam Room C112
Medication Room and C124 Bedroom. The sharp containers can not be more
then 3/4 full at any time. Contractor is to ensure all liners, labels
and containers are in place prior to October 1, 1997. Point of contact
at Center for Aging will be the Admin. Officer Virginia Largent.
Provision 52.212-1 INSTRUCTIONS TO OFFERS-COMMERCIAL ITEMS,- COMMERCIAL
ITEMS IS APPLICABLE. The Government will evaluate offers for award
purposes by adding the total price for all options to the total price
for the basic requirement. The Government may determine that an offer
is unacceptable if the option prices are significantly unbalanced.
Evaluation of options shall not obligate the government to exercise the
options(s). Offeror must submit a completed copy of provision 52.212-3,
OFFEROR REPRESENTATIONS AND CERTIFICATIONS -- COMMERCIAL ITEMS with its
quotation. Clause 52.212-4, CONTRACT TERMS AND CONDITIONS is applicable
to this acquisition. Clause 52.212-5, CONTRACT TERMS AND CONDITIONS
REQUIRED TO IMPLEMENT STATUTES OR EXECUTIVE ORDERS -- COMMERCIAL ITEMS,
applies to this acquisition. In addition, all applicable clauses and
provisions from the VA Acquisition Regulation apply. This is a small
business set-aside, sic code is 4953 size standard 6m. Quotations are
due to the Contracting officer (90C), 300 Ala Moana Blvd., Room 3119A,
Honolulu, HI 96813, no later than 2:00PM local time August 7, 1997.
