Freeman, Contracting Officer, (216) 838-6091 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation 541-186-97 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-46. This is a sole source procurement under the Standard Industrial Classification (SIC) Code 5047 and Small Business Size Standard of 500 Employees. The VA Medical Center, Brecksville, OH intends to negotiate sole source with Althin Medical, 15620 NW 60th Avenue, Miami Lakes, FL 33014-9308, for procurement of 103 cases of the Altra Flux 200G Hemodialyzer, stock number 211-720G, packaged twelve (12) each per case. Contractor shall meet the following requirements: Salient characteristic: Meltspun cellulose diacetate membrane which offers biocompatibility, provides effective removal of a wide range of toxic solutes including beta 2 micro-globulin with balanced hydraulic permeability. Performance characteristics are provided in VIVO (real life); design tubular in nature, 9.5" inch height X 2" inch circumference; outside shell polycarbonate (medical grade plastic). Item must be compatible with our existing Althin 1000 Dialyzer System. Delivery shall be Free on Board (FOB) Destination within five (5) days of award. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial items applies to this acquisition with the following changes, items (e) and (h) of the standard clause are deleted. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial items, with their offer. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses: 52.203-6, 52.203-10, 52.219-8, 52.219-14, 52.222-26, 52.222-35, 52.222-36, 52.222-37 and 52.225-3. Warranty of said product shall be in accordance with standard industry practice in effect at time of award. Interested offerors may identify their interest and capability to meet the requirements by submitting a capability statement which must contain adequate information on supplying this type of system, and qualification to provide the product. This notice of intent is not a request for competitive proposals. However, all capability statements received within five (5) calendar days after the date of publication of this synopsis will be considered by the Government. (0192)
