Q -- COMMUNITY BASED OUTPATIENT CLINIC SOL RFP No. 636-70-97 DUE 082297 POC Karen Goodell, Contracting Officer (90C), 402-449-0613 Contractor to provide all necessary board certified physicians and staffing required to provide a Community Based Outpatient Clinic (CBOC) within the geographical location of northeast Nebraska for periodic medical exam services as required for an estimated 1300 veteran beneficiaries of the Department of Veterans Affairs Medical Center, 4101 Woolworth Avenue, Omaha, Nebraska 68105, for the period commencing October 1, 1997, through Septemper 30, 1998, and four (4) subsequent renewal periods. Patient referrals will include eligible VA beneficiaries residing within the following counties in the state of Nebraska: Antelope, Boone, Boyd, Cedar, Colfax, Cuming, Dakota, Dixon, Garfield, Greeley, Holt, Knox, Madison, Nance, Pierce, Platte, Stanton, Thurston, Valley, Wayne and Wheeler. Scope of work is to provide primary care services to eligible veterans through a contract arrangement with a local health care provider or a medical school affiliate as required and in accordance with JCAHO standards. The Omaha VA Medical Center will serve as the parent facility for the CBOC. RFP No. 636-70-97 has been assigned to this solicitation and will be available approximatley July 23, 1997.
