requirement to STEAM CLEAN CARPETS BY HOT WATER EXTRACTION AND DRY
CHEMICAL EXTRACTION BASE WIDE. 17(i). THIS IS A COMBINED
SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH
THE FORMAT IN FAR SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL
INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE
ONLY SOLICITATION; PROPOSALS ARE BEING REQUESTED AND A WRITTEN
SOLICITATION WILL NOT BE ISSUED. 17(ii). Solicitation #F33601-97-B-0025
is an Invitation for Bid (IFP). 17 (iii). This solicitation document
and incorporated provisions and clauses are those in effect through
Federal Acquisition Circular 90-46. 17(iv). This is an unrestricted
acquisition. The associated standard industrial classification (SIC)
code is 7217. The small business size standard in $2.5M. 17(v). This
acquisition has two line items for the base and optional contract
periods: LINE 1 is for HOT WATER EXTRACTION and LINE 2 is for Dry
Chemical Extraction.The period of performance is 1 October 1997 -- 30
September 1998. Four one-year options may be awarded. Bids will be
provide for each line item for each year, to include price per square
footage, for the base and optional periods (totaling five years). All
work will be in accordance with the following Performance Work
Statement for Carpet and Upholstery Cleaning for Areas A, B, and C
dated 1 October 1997. SECTION TITLE C-1 GENERAL INFORMATION C-2

GENERAL INFORMATION 1.1 SCOPE OF WORK: The contractor shall provide
complete carpet and upholstery cleaning services to include all
personnel, equipment, tools, materials, transportation, supplies,
supervision, and other items and essential services to perform in
accordance with the Performance Work Statement (PWS) of this contract
basewide on Wright-Patterson AFB, Ohio. Note: Carpet to be cleaned
includes floor carpet and wall carpet to a height of six (6) inches
above floor level. 1.2 Contractor 1.2.1. Contract Manager: The
contractor shall provide a contract manager who shall be responsible
for the performance of the work. The name of this person and an
alternate(s), who shall act for the contractor when the manager is
absent, shall be designated in writing to the Contracting Officer
within 10 days after Contract Award. 1.2.1.1. The contract manager or
alternate shall have full authority to act for the contractor on all
contract matters relating to daily operation of this contract on the
installation. 1.2.1.4 Supervisors. The contractor shall provide a
minimum of one (1) working supervisor. The supervisor shall have a
minimum of two (2) years experience in carpet and upholstery cleaning.
The supervisor shall insure competent and timely performance of the
work. In the event that the Contract Manager is not immediately
available, the supervisor shall have the authority to act as the
Contract Manager for the contractor. The contractor shall provide for
a method by which the supervisor can be contacted at all times within
15 minutes by the Contracting Officer or by the Government
Representative (GR). The supervisor shall coordinate all work with the
Real Property Building Manager (RPBM) and the (GR). 1.2.2. Employees:
1.2.2.1. Contractor Employees: The contractor shall not employ persons
for work on this contract if such employee is identified to the
contractor by the contracting officer as a potential threat to the
health, safety, security, general well-being or operational mission of
the installation and its population. 1.2.2.2. Contractor personnel
shall present a neat and clean appearance. The contractor shall furnish
an identifying badge, which shall include as a minimum, the person's
name and the name of the contractor. Each employee shall wear the badge
on the outer clothing on the front of the body between the neck and
waist and such badge shall be visible at all times. 1.2.2.3. Employee
Qualifications: The contractor shall insure that employees have the
following current and valid professionalcertifications before starting
work under this contract. Contractor personnel performing work under
this PWS shall have as a minimum 1 year experience in carpet and
upholstery cleaning. Contractor personnel shall be knowledgeable of the
operational requirements of equipment and cleaning techniques.
Personnel shall have a current working knowledge through training and
experience to accomplish scheduled cleaning as well as on-call
emergency cleaning. 1.2.2.4. The contractor shall submit to the
Contracting Officer a list of intended contract personnel and their
qualifications as listed in paragraphs 1.2.1. and 1.2.1.4. above as
part of the bid process. 1.2.2.5. The contractor shall not employ any
person who is an employee of the United States Government if employing
that person would create a conflict of interest. Additionally, the
contractor shall not employee any person who is an employee of the
Department of the Air Force, either military or civilian, unless such
person seeks and receives approval in accordance with Joint Ethics
Regulation, DOD 5500.7R. In addition, the contractor shall not employ
any person who is an employee of the Department of the Air Force if
such employment would be contrary to the policies in Air Force
Instruction 64-106 Air Force Industrial Labor Relations Activities.
1.2.2.6. The contractor is cautioned that off duty active military
personnel hired under this contract may be subject to permanent change
of station (PCS), change in duty hours or deployment. Military
Reservists and National Guard members shall be subject to recall to
active duty. The abrupt absence of these personnel could adversely
affect the contractor's ability to perform, however, their absence at
any time shall not constitute an excuse for nonperformance under this
contract. 1.2.2.7. The contractor is prohibited from employing off-duty
Government personnel who are surveilling any contracts or subcontracts
awarded to the contractor. 1.2.2.8. Control of Contractor Employees.
The selection, assignment, reassignment, transfer, supervision,
management, and control of contractor employees in performance of this
work statement shall be the responsibility and prerogative of the
contractor. The contractor shall comply with the requirement set forth
in the performance work statement. The contractor shall comply with
regulations of Wright-Patterson AFB concerning conduct of employees as
referenced herein. When the government directs, the contractor shall
restrict the employment under the contract or remove from performance
on the contract any person who is identified as a potential threat to
the health, safety, security, or operational mission of the
installation and its population. 1.2.2.9. Contractor personnel shall
comply with directives pertaining to operation of privately owned
vehicles on Wright-Patterson AFB (per AFR 125-14 Motor Vehicle Traffic
Supervision). 1.2.3. SECURITY REQUIREMENTS. 1.2.3.1. The contract
manager or alternate shall complete a "Request for Identification
Credential (AFMC Form 496)" for each employee of the contractor
requiring access to Wright-Patterson Air Force Base. The request shall
be submitted to Pass and Registration (Bldg. 274). The government
shall provide a completed "Identification Credential (AFMC Form 387),"
which shall be issued, displayed and surrendered as directed in AIR
FORCE INSTRUCTION 131-209, The Air Force Resource Protection Program.
1.2.3.2. The contractor shall be responsible for assuring that
contractor's employees comply with all security requirements imposed by
the local commander at all times while his employees are on the
installation, and shall follow all instructions of the local
organizational commander having jurisdiction over the area being
serviced in all matters pertaining to security. The contractor and his
personnel shall be required to coordinate and schedule all work in
classified areas with the appropriate government representative prior
to entering these areas. 1.2.3.3. The contractor shall not be granted
access to classified information, but shall be required to perform
duties in controlled areas where classified information is processed
and stored. Should the contractor inadvertently gain access to
classified information, he will immediately notify the Security Desk at
257-6516. 1.2.3.4. Lost and Found Property: It is the responsibility of
the contractor to insure that all articles of personal or monetary
value found by the contractor's employees are turned in to the Security
Police. 1.2.4. Employee Training. Contractor shall provide adequate
training and instruction for personnel to perform designated tasks.
Employees must be able to read, write, and understand English
sufficiently to comprehend written and spoken instructions. 1.3.
QUALITY ASSURANCE. The government will evaluate the contractor's
performance using one-hundred percent (100%) surveillance. Government
personnel will record all surveillance observations. When an
observation indicates defective performance, the Government
Representative (GR) will advise the contract manager or representative.
The AF Form 372 Contract Monitoring and Surveillance Report will be
delivered to the contractor's office for initialing later the same day.
The initialing of the observation does not necessarily constitute
concurrence with the observation, only acknowledgment that he or she
has been made aware of the defective performance. Government
surveillance may occur at anytime during the performance period of this
contract. Such surveillance will be accomplished according to standard
inspection procedures or other contract provisions. Any action taken
by the contracting officer as a result of surveillance will be
according to the terms of this contract. 1.3.1. Performance Evaluation
Meetings. The contracting officer may require the contract manager to
meet with the contracting officer, contract administrator, Government
Representative, and other government personnel as deemed necessary.
The contractor may request a meeting with the contracting officer when
he or she believes such a meeting is necessary. Written minutes of any
such meetings shall be recorded in the contract and signed by the
contract manager and the contracting officer or contract administrator.
If the contractor does not concur with any portion of the minutes, such
nonoccurrence shall be provided in writing to the contracting officer
within 10 calendar days following receipt of the minutes. 1.4.2. Work
Scheduling. See Section C-5, paragraphs 5.3. 1.4.3. QUALITY ASSURANCE
EVALUATOR 1.4.3.1. The Government Representative will be a
representative of the Contracting Officer and will participate in the
administration of this contract except where exemptions have been
provided by the Functional Area Chief and the Base Contracting Officer
in accordance with AFM 64- 108 (Service Contracts), paragraph 1.4.
Subsequent to contract award, the identity of the Government
Representative, with a brief resume of their duties and authority will
be promptly furnished to the successful bidder/offeror. 1.4.3.2. The
Government Representative will inform the contractor when discrepancies
occur and will request corrective action. The Government Representative
or alternate will make a notation of the discrepancy on their tally
checklist and surveillance report with the date and time the
discrepancy was noted and will request the contract manager (or
authorized representative) to initial the entry on the surveillance
report (AF Form 372). 1.4.3.3. Any matter concerning a change to the
scope, prices, terms or conditions of this contract shall be referred
to the Contracting Officer. 1.4.3.4. The services to be performed by
the Contractor during the period of this contract shall at all times
and places be subject to review by the Contracting Officer or
authorized representative. 1.4.3.5. Meetings may periodically be held
between top level base personnel and contractor management to discuss
contract status. The contractor shall be notified in writing at least
five days in advance of the place and time of required meetings. 1.6.
HOURS OF OPERATION. 1.6.1. Normal Duty hours: Wright-Patterson's normal
duty hours are 0715 (7:15 am) to 1600 (4:00 pm). 1.6.1.1. Recognized
Holidays: The contractor is not required to provide service on holidays
recognized by the government. 1.6.1.2. Unforeseen Base Closures. When
an unforeseen base closure occurs on a scheduled day of work, the
Government shall have the following options for each type of service:
1.6.1.2.1. To require the contractor to perform the work on the
following day, unless the following day is a Saturday, Sunday, or
holiday and routine work is not scheduled for those days. 1.6.1.2.2 To
forego the work and reduce payment due the contractor accordingly for
work not performed. 1.6.1.2.3. To reschedule the work on any day
satisfactory to both parties. 1.6.1.3.2. The base could be closed
because of, but not limited to, adverse weather, a base or a local
disaster, security problems or other events that would necessitate the
closing of the base. Should one of these situations occur the
Contractor should listen to or watch one of the radio or television
stations for notification of a possible base closure. 1.6.1.3.3. The
Contractor will not receive any other form of notification of a base
closure from the Government. The Contractor is responsible for the
notification of his or her employees. 1.7. CONSERVATION OF UTILITIES.
The contractor shall instruct employees in utilities conservation. The
contractor shall be responsible for operating under conditions that
prevent the waste of utilities, which shall to include the following:
1.7.1. Lights shall be used only in areas where work is actually being
performed. 1.7.2. Employees shall not adjust mechanical equipment
controls for heating, ventilation, and air conditioning systems. 1.7.3.
Water faucets or valves shall be turned off when not in use. 1.7.4.
Government telephones shall not be used for any personal, toll, long
distance or off-base calls. SECTION C-2 DEFINITIONS 2.1. GENERAL
DEFINITIONS. 2.1.1. Defective Service. A service output that does not
meet the standard of performance specified in the contract for that
service. 2.1.2. Quality Assurance. A planned and systematic pattern of
all actions necessary to provide confidence that adequate technical
requirements are established; products and services conform to
established technical requirements; and satisfactory performance is
achieved. For purposes of this contract, quality assurance refers to
actions by the government. 2.1.3. Government Representative (GR). A
functionally qualified government person who performs quality assurance
functions for a contracted service. 2.1.4. Chief Quality Assurance
Evaluator. (Chief QAE) A government employee who is responsible for all
GR's, their actions and standards. 2.1.5. Defect. Any nonconformance
with requirements specified in the PWS. 2.1.6. Service. A job which
calls directly for a contractor's time and effort rather than for a
concrete end product. The service is to be performed to the standard
and within the performance requirement. The contractor must do the
specific job, and meet the standard and the performance requirement
before one can say the performance has been acceptable and that he or
she should be paid. 2.1.7. Hazardous Material. Any substance or
material, in any quantity or form, which because of its quantity,
concentration, physical, chemical, or infectious characteristics shall
pose a substantial hazard to human health or the environment or
material. Specific hazardous substance definitions shall be found in
OSHA 29 CFR 1910.1000, sub-part Z, Toxic and Hazardous Substance; EPA
40 CFR Chapter 1, Table 302,4 Section 302 of SARA Title III, and DOT 49
CFR Section 172.101. Hazardous federal stock class numbers and their
definitions can be found by using Federal Standard 313, Table I and II,
in lieu of specific information. 2.1.8 Call Number: A sequential number
assigned by the Contract Administrator at the time of contractor
notification of services required. 2.2. TECHNICAL DEFINITIONS PECULIAR
TO THIS PWS. 2.2.1. Real Property Building Manager (RPBM). The
government employee who is assigned the responsibility of insuring that
his/her building or area of building is maintained in an efficient
safe, clean and orderly condition. The contract manager or supervisor
shall coordinate all cleaning services with the RPBM. The RPBM shall
monitor the services and report discrepancies to the appropriate
Government Representative immediately, as a customer complaint. SECTION
C-3 GOVERNMENT FURNISHED PROPERTY AND SERVICES 3. GENERAL INFORMATION.
The government shall provide, without cost, the facilities equipment,
materials and services listed below 3.1. Government-Furnished
Property: (RESERVED 3.1.2. Government-Furnished Materials: (RESERVED)
3.2. GOVERNMENT-FURNISHED SERVICES 3.2.1. Utilities: The government
will furnish hot and cold water,electricity and sanitary sewers, when
available, at no cost to the contractor for the sole purpose of
accomplishment of this contract and shall not be used for any other
purpose. 3.2.2. Telephone: Pay phones are located in some facilities to
be used by the contractor. 3.2.3 Fire Protection: The government will
provide on-base fire prevention and protection. The Fire Department
emergency phone extension is 911 for emergencies, 257-6767 for routine
calls. 3.2.4. Emergency Medical Services: The government shall provide
emergency medical treatment and emergency patient transportation
service for contractor personnel. The contractor shall reimburse the
government for the cost of medical treatment and patient transportation
service at the current inpatient or outpatient treatment rate, as
appropriate. The emergency phone number for medical service is 911.
3.2.5. Security Police: The government will provide general on-base
Security Police service and protection. Security Police phone extension
is 911 for emergencies, 257-6516 for routine calls. SECTION C-4
CONTRACTOR FURNISHED ITEMS AND SERVICES 4. GENERAL Information. Except
for those items or services specifically stated to be Government
furnished in Section C-3, the contractor shall furnish everything
required to perform this contract according to all its terms. 4.1.
Equipment. The contractor shall provide all equipment and tools
required to perform the services of this contract. All contractor
equipment and tools shall be identified to distinguish contractor
equipment from government equipment. All electrical equipment used by
the contractor shall meet all safety requirements of this contract and
shall be UL approved. It shall be the contractor's responsibility to
assure that damaged, worn, or broken equipment is repaired or replaced
immediately. All electrical equipment shall have 3 prong grounded
plugs or shall be double insulated. 4.1.1. The Government shall not be
liable or responsible for any supplies or materials lost, stolen,
damaged or destroyed for any reason. This shall be the sole
responsibility of the contractor. 4.1.2. The contractor shall be
required to maintain a suitable system of records accounting for all
services on this contract. 4.1.3. The contractor shall transport
personnel, materials, and equipment in contractor maintained and
operated vehicles, as necessary in the performance of this contract.
