INTRODUCTION: The Social Security Administration (SSA) is interested in receiving quotations on the purchase of VHS Format video tapes on Workplace Violence listed below to be shipped to Social Security offices throughout the United States. This is a combined synopsis/request for quotation for commercial items prepared in accordance with FAR Subpart 12.6. In the event that a contractor can provide mutiple items which are listed herein, in excess of $100,000, the award will be in accordance with FAR 13.6 Test Program for Certain Commercial Items as supplemented with additional information included in this notice. This is the only announcement; quotations are being requested and a written request for quotation will not be issued. This is a simplified acquisition, 100% small business set-aside, issued as a request for quotation (RFQ), number SSA-RFQ-97-3117. Quotations submitted from other than small businesses will be rejected. All responsible small business sources may submit a quotation which shall be considered by the agency. The standard industrial classification (SIC) code for this acquisition is 3669. The small business size standard is 750 employees. This request for quotation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-46.
