Moreland, Contracting Officer, (317)267-8709, FAX (317)267-8709 This is a combined synopsis/solicitation for commercial item prepared in accordance with the format in subpart 12.6 as supplemented with additional information including in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation No. 583-97-3-6970-0087 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-42. Contractor to furnish and install Nurse Call System in existing Eight East, Psychiatric Clinic (locked ward)at the VA Medical Center, 1481 West Street, Indianapolis, IN. Visits to the site by offerors is recommended to see and understand the working environment. Site visit is scheduled on June 17, 1997 at 10:00 a.m. Offerors are to report to Bldg. 1, Room A-B040 for site visit. The contractor will need to exercise caution while working and always secure all tools and hardware due to thetype of patients that will be in the area. The contractor may be required to work in the same area as VA Tradesmen and other contractors. Work in individual bedrooms will require working on one room at a time with gaps of time (2-3 weeks) between rooms. Contractor shall also provide two (2) operators manuals, one (1) service manual, one (1) schematics, and one (1) parts list and ordering information. Delivery and installation shall be made within sixty days from receipt of order. The provisions at 52.212-1, Instructions to Offerors -- Commercial, and 52.212-2 Evaluation -- Commercial Item, apply to this acquisition. The following factors shall be used to evaluate offers: (1) price, (2) warranty, (3) delivery time, and (4) technical (capibility of the offered system to meet agency needs). Offerors shall include a completed copy of the provision at 52.212-3 Offerors representations and Certification -- Commercial Items, with its offer. 52.212-4 Contract Terms and Conditions -- Commercial Items, 52.212-21 Descriptive literature, and 52.232-33 Mandatory Information for Electronic Funds Transfer Payment, applies to this solicitation.
