St. Mary's Road, Tucson, Az 85745; Carle Clinic, 500-92-0053 110 West University Avenue, Urbana, IL 61801; Visiting Nurse Service of New York, 500-92-0054, 107 E. 70th Street, New York, New York, 10021-5087 to complete additional assessment interviews for the extension year. In accordance with FAR 6.302.1 HCFA intends to negotiate with the four (4) sites to continue the operation and current data collection activities necessary to effectively conduct the CNO demonstration. The Demonstration will provide policy in response to the Administration's requirement and Agency Needs. The period of performance will be an additional 12 months.
