synopsis/solicitation for commercial services prepared in accordance
with the format in Subpart 12.6, as supplemented with additional
information in this notice. This is a Request for Quotation (RFQ).
Responses are due May 28, 1997, 4:30 p.m. CST at the VA Medical Center,
Attn: Contracting Officer (90C), 1011 Honor Heights Drive, Muskogee, OK
74401. Provide all travel, equipment, materials, and labor to Retube
the Condenser Bundle in a York Model HT-510 Low Pressure Water Chiller.
Please offer a quote in one (1) lump sum total. 1 JB $__ __ __ __total.
REQUIREMENTS: The contractor shall disassemble, retube, and reassemble
the condenser bundle and perform a standing vacuum test on the
condenser of the York HT-510 chiller. All parts must meet York factory
specifications and all work must be completed in accordance with the
manufacturer's specifications. Contractor shall be aware of physical
chiller location relating to access, removal, disassembly/reassembly of
components, setup and rigging requirements, and mechanical room layout.
Contractor shall be responsible for understanding and compliance with
jobsite safety rules, building work rules, and time of day access, and
the establishment of accident emergency contacts. Contractor shall be
licensed and regularly engaged in performing the specified work.
Contractor shall furnish satisfactory written evidence with quotation
of prior experience in large centrifugal chiller maintenance and
repair, specifically the type of work described herein, providing at
least three (3) references for verification. Contractor shall provide
all necessary labor, rigging, and miscellaneous materials, and any
special tools and services to perform the chiller retube work.
Contractor shall submit a work schedule and time frame for completion
with quotation. Contractor is responsible for clean up of area and
removal and offsite disposal of tube material. Contractor shall provide
a warranty to componentssupplied as a part of the scope of work for a
minimum of one (1) year. Labor performed during the course of the
chiller retube shall also be warranted for a minimum of one (1) year.
The names of technicians who will perform the work shall also be
furnished with the quotation. The Service Contract Act of 1965 applies.
Department of Labor wage rates for 23400 Heating, Refrigeration and Air
Conditioning Mechanic $15.39 per hour, and 11180 Laborer $5.85 per hour
apply to this solicitation. In accordance with Part 12 of the Federal
Acquisition Regulations the following terms and conditions are in
