Provide name, address, P.O.C. & telephone number of manufacturer of meat & cheese. Commodity offered must also meet the following standards: Frozen bagged meal kits must have a six months frozen shelf life after date of delivery, must have a 14 day refrigerator shelf life and must be able to remain fresh and untainted for a minimum of 4 hours after being removed from the freezer but not in a refrigerator.
