Waterman (520) 629-4623 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This annoucement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Government intends to enter a sole source agreement with St. Jude Medical Inc., One Lillehei Plaza, St. Paul, Minnesota 55117, for Mechanical Heart Valve SJM Masters Series Valves. Solicitation #678-23-97 is issued as a request for proposal (RFP). The following minimum characteristics have been identified 1) Aortic Valve, Rotatable Standard Cuff, Polyester; and 2) Aortic Valve, Rotatable Expanded Cuff, Polyester. Estimated annual usage of five Heart Valves. Required delivery schedule is 30 days from date of contract award. The contract is for base year and four option years. Items will be delivered to VAMC Tucson, 3601 S. 6th Avenue, Tucson, AZ 85723. Provision 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and any addenda to the provision. The following factors will be used to evaluate offers: a) technical capability of the item offered to meet the Government requirement; and b) price. A completed copy of the provision 52.212.-3, Offeror Representations and Certifications -- Commercial Items are to be included in your submission. Clause 52.212-4, Contract Terms and Conditions -- Commercial Items applies to this acquisition along with any addenda to the clause. Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutues or Executive Orders -- Commercial Items, applies to this acquisition in addition to the following FAR clauses a) 52.203-10, Price or Fee Adjustment for Illegal or Improper Activity (41 U.S.C.) ; and b) Buy American Act -- Supplies (41 U.S.C. 10). The Government reserves the right to use a Purchase Card to pay for items acquired from this solicitation.
