synopsis/solicitation for commercial services prepared in accordance
with the format in Subpart 12.6, as supplemented and additional
information included in this notice. This solicitation is being issued
as a Request for Quote (RFQ). All RFQ's are due on 4/08/97, 4:00 p.m.,
local time, Department of Veterans Affairs Medical Center, A&MMS,
(90C), 1011 Honor Heights Drive, Muskogee, OK 74401 or if hand carried
to: Contracting Officer, 3105 Azalea Park, Room 109, Muskogee, OK
74401. This announcement constitutes the only solicitation; quotes are
being requested and a written solicitation will not be issued. This is
a request for quotation due to the fact that the estimated dollar
limitation is below the Simplified Acquisition Threshold of
$100,000.00. The solicitation document and incorporated provisions and
clauses are those in effect through Federal Acquisition Circular
90-45. The SIC Code for this procurement is 8049 and the small business
size is 3.5 mil. DESCRIPTION, QUANTITY, UNIT, UNIT PRICE, TOTAL:
Provide Primary Care services in accordance with Requirements and terms
and conditions of resulting contract at the Tulsa Outpatient Clinic
(TOPC), 635 W. 11th, Tulsa, OK., 74127. Service to begin 14 April 1997
through 27 July 1997. Price to include all personnel, services,
travel, per diem, etc., per a one week period. Physician services will
not be required on weekends or Federal Holidays. A week will be
considered Monday through Friday 8:00 a.m. to 4:30 p.m. (1 FTE),
estimated 12 weeks @ $__ __ ____ week for a total cost of $__ __ ____.
REQUIREMENTS: The primary care physician shall be responsible for
providing comprehensive health care to the patients presented for
evaluation/treatment in the primary care team and will be under the
direction of the Chief Medical Officer of the Tulsa Outpatient Clinic.
These requirements and any applicable terms and conditions shall be
included in any subcontracts. a. Evaluation of patients: Evaluates
patients who are presented for care which includes the following: (1)
A detailed and complete history and physical will be accomplished
consistent with the patients presentation. (2) Orders and interprets
appropriate diagnostic studies. (3) Development of a plan of care. (4)
Writes appropriate orders and/or prescriptions. (5) Establishes plans
for follow-up care including appropriate consultations and
appointments. (6) Effective communication of plan of care with patient
and/or family. (7) Assures appropriate disposition of patient,
admission, transfer to another facility or discharge to home. (8)
Establish communication in the medical record to the physician to which
the patient is referred or transferred. b. Admission of patients which
includes the following: (1) Complete and detailed history physical
examination. (2) Initial diagnosis of patient's condition. (3) Write
initial admission orders consistent with theinitial care plan. 2. Other
Services will be performed as follows: a. Charting requirements:
Documentation of all care provided and planned or recommended including
as appropriate, medical prescriptions. This information will be entered
in each patient's medical record with a frequency that allows for
timely implementation of medical plans and reflection of care provided.
This includes as a minimum, one timed note for each episode of care. b.
Consultation: Consultation will be provided as needed to nursing staff,
ancillary staff and medical staff to assure timely, safe and
