Services to be rendered is entitled "Veterans Five-Year Health Care Follow-up Project -- Phase I". Contractor shall provide survey follow-ups of a group of veterans who received health services at Department of Veterans Affairs Medical Centers throughout the United States who participated in a health services evaluation project and who completed an intake form essentially comparable to the follow-up form they will be asked to complete for this project.
