The purpose of this procurement is to supplement FCS' ability to visit food stores in person, for the purpose of confirming that the store is eligible for authorization according to program regulations. Services to be performed by the contractor will include participation in FCS sponsored training sessions, and for each store visit: completion of a survey form containing descriptive information on the store's condition; photographs of the store's food and non food sections; and providing a sketch of the store layout. In addition, the contractor will be required to submit store visit reports within prescribed timeframes and provide monthly management summary reports. Proposals will be solicited to perform contract services in one or more of the seven FCS regions. The contract(s) will for a base yearand option year.
