BEDNAR (717) 895-6369 Tobyhanna Army Depot is soliciting for services for an on-site (on depot) full time program for functional capacity evaluations, work site ergonomic evaluations of work methods and tasks and physical therapy to rehabilitate and condition injured employees. The services will apply to employees in an industrial setting where the following types of tasks are performed: sanding, painting, paint spraying, cable pulling, repetitive motion, and metal working operations. The government will provide the necessary equipment for the on-site service facility. Period of performance is twelve months with option for up to four additional twelve month periods. DAAC71-97-R-0003 applies; approximate issue/closing dates are 21 Jan/14 Feb 97. These dates are only estimated. Exact dates will be established when the solicitation is actually prepared and issued. This action is being solicited using FAR part 12 commercial item/services procedures. Although this solicitation is unrestricted, sources requesting a copy of this action should identify their business size when making the request. SIC is 8099 with $5 million in average annual receipts. All responsible sources may submit an offer which will be considered. (0003)
