J -- MAINTENANCE AND REPAIR GE CT SCANNER SOL 590-10-97 DUE 122396 POC Michale Weaver (757) 728-3116 Furnish all parts (except x-ray tube and housings), labor and travel to provide on-site maintenance and repair of one General Electric Computerized Tomographic System, Model 9800 Hispeed Advantage, located at the Department of Veterans Affairs Medical Center, Hampton, Virginia. System software maintenance will be included in the awarded contract. Contract will include emergency repair services. Normal hours of coverage are 8:00 a.m. to 5:00 p.m., Monday through Friday, excluding Federal Government holidays. Contractor's service personnel must be on site making repairs within four hours of notification that repairs are required. Monthly preventive maintenance inspections will be required and must be done between the hours of 5:00 p.m. to 9:00 p.m., Monday through Friday. Contract period will be January 1, 1997 through September 30, 1997 with two one year option periods. The contract will be awarded to a single prime contractor as a firm fixed price contract. Offer will be evaluated based on the following criteria: (1) Company experience and demonstrated capability; (2) personnel qualifications; (3) understanding of problem and project approach; and (4) price.
