Officer (908)532-4024. Pharmacist to work at Fort Monmouth, NJ and Fort
Hamilton, NY on a ''as needed basis.'' This is a combined
synopsis/solicitation for commercial items prepared in accordance with
the format in Subpart 12.6 as supplemented with additional information
included in this notice. This announcement constitutes the only
solicitation; Proposals are being requested and a written solicitation
will not be issued. The solicitation document and incorporated
provisions and clauses are in effect through Federal Acquistion
Circular 90-37. This acquisition is 100% set aside for small business.
The Standard Industry Code (SIC) is 8093. FOB Point is destination.
Provision at 52.212-1, instructions to offerors-commercial, applies to
this acquisition. Provision at 52.212-3, offeror representations and
certifications-commercial items with its offer. Clause at 52.212-4,
contract terms and conditions-commercial items, applies to this
acquisition. Also the clause at 52.212-5, contract terms and conditions
required to implement statues or executive orders-commercial items
applies. The following FAR Clauses are applicable under clause
52.212-5; 52.219-8; 52.219-9; 52.222-26; 52.222-35; 52-222-26;
52.222-37; 52.222-41; 52.222-42; 52.222-43; and 52.222-44. The required
hours for the acquisition are 8:00am to 5:00pm (approximately 1000
hours or 125 days), beginning 18 Nov 96 thru 30 Sept 97 (week days
only, on call, one week lead time). Pharmacist must speak fluent
english and have a valid license in any of the 50 states. The
pharmacist is responsible for the operation of the outpatient pharmacy.
Reads and interprets prescriptions. Duties also include advising
patients on how to properly take their medications. Answers all patient
questions concerning medications or pharmacy policies. Provides
appropriate information in response to questions from physicians,
nurses and other clinic staff. Use's pharmacy computer system for all
phases of prescription filling. Proposals for this acquisition are due
