G -- LAKELAND INSTITUTE COOPERATIVE AGREEMENT SOL 541-88-97 DUE 102896 POC matt t. verhulst The Cleveland VAMC will enter into a cooperative agreement with the Lakeland Institute based in the Lorain/Elyria Ohio area for space and services. Lakeland Institute is authorized by the Addiction Recovery Board of Lorain County, Ohio, to provide primary outpatient and continuing care services to indigent adults and is established within the community. The VA will utilize such services rendered to veterans, spouses and family members who are, in large part, of similar socioeconomic status to those regularly treated by Lakeland. This cooperative agreement incorporates the availability of detoxification services to individuals at the joint location if required in order to provide easy access to referrals and earlier intervention/coordination of care. Collateral services will be provided on a limited basis under the same agreement/by the same provider to the Ohio Veterans Home in Sandusky, Ohio at an established location in Sandusky, Ohio. Matt T. Parties may identify their interest in and ability to meet the requirements for all services identified above by submitting qualification information which conclusively demonstrates such capability.
