combined synopsis/solicitation for commercial services prepared in
accordance with the format in FAR Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation, proposals are being requested and a
written solicitation will not be issued. The solicitation number is
F04700-96T7637. This is a Request for Proposal. The solicitation
document and incorporated provisions and clauses are those in effect
through FAC 90-40 and DAC 91-10. This acquisition is 100% small
business set aside. Standard industrial classification code: 7699.
Small Business Size Standard: $5.0M. The acquisition includes one line
item for test, maintenance and overhaul of Scott model 2.2 and 4.5 MSA
Ultralite and Interspirolite Breathing Apparatus according to
manufacture's recommendations to include: one year functional check on
breathing valve, two year functional check on pressure reducers,
overhaul, all test results, Pass or Fail, in writing. Upon request,
replacement of mask, and, supply and install donning switches to
regulators, communications systems, Inspirohatch on mask, and, replace
rubber harness on mask with webbing. Accomplish all the required
tests, maintenance, and overhaul of SCBA & DOT cylinders, both high
pressure (4500 psi) and low pressure (2000 psi) to include: hydrostatic
testing of cylinders (at the DOT's required interval), check all
cylinders for leaks after being hydrostatically tested, fill SCBA
cylinders with breathing air, paint cylinders white with a 2''
reflectorized green band below cylinder neck (upon request only),
insert clear plastic sleeve around cylinder body to protect painted
surface (upon request only), provide all test results, Pass or Fail, in
writing. When directed by user provide a 5-day turnaround time from
delivery of apparatus until completed. Normal turnaround time will not
exceed two weeks. Contractor must be certified by Scott and Interspiro
to perform all work. Period of performance for this contract is 1 Oct
96 through 30 Sep 97 with the Government having the options to extend
the contract for four years. FAR provision 52.212-1, Instructions to

contract value to include all options. Offerors must include a
completed copy of FAR provision 52.212-3, Offeror Representations and
Certifications--Commerical Items, with the offer. FAR clause 52.212-4,

52.212-5, Contract Terms and Conditions Required to Implement Statutes
