Synopsis/solicitation for commercial items prepared in accordance with
the format in Subpart 12.6, as supplemented with additional
information included in the notice. This notice constitutes the only
solicitation: Proposals are being requested and ''Written
Solicitation'' will not be issued. Solicitation 689-143-96 is issued as
a request for proposal and incorporates provisions and clauses that are
in effect through Federal Acquisition Circular 40. Supply and install
the following additional equipment for a Teleradiology Communication
System at the VA Connecticut Healthcare System, West Haven and
Newington Campuses. Equipment shall be brand name or equal and
compatible with the following equipment supplied by E-Systems Medical
Electronics, 115500 IH West Suite 200, San Antonio, TX 78230. Two (2)
PACSPRO/DX-2 Diagnostic View Station including 2 Megascan 2048x2560 X
12 bit display monitors; three (3) StatView/DX-2M Clinical Viewstations
including 2 Megascan 1280x1600 display monitors; one (1) EMED CR
Gateway converts the FUJI CR digital image data into DICOM 3.0 format
and transfers the data for distribution, display and archiving; one (1)
CISCO 2501; one (1) network file server upgrade to increase short term
storage capacity to support image acquisition workflow; one (1)
INT-DHCP Bi-directional interface for the DHCP database; EMED/PACS
Archive including archive host and archive management software and
licenses DISC 255 jukebox and two (2) 5.25 optical disk drives; and 255
OPT 5.25 optical discs. Standard Industrial Classification Code (SIC)
#5045, Computer, Peripheral Equipment and Software shall apply to this
procurement. The provisions at 52.212-1, Instruction to
Offerors-Commercial Items; 52.212-2, Evaluation-Commercial Items,
52.212-3, Offers Representations and Certifications-Commercial Items;
52.212-4, Contract Terms and Conditions - Commercial Items apply to
this acquisition. The following factors in priority of relative
importance shall be used to evaluate offers: (1) Ability to provide
bi-directional interface with VA Connecticut's DHCP computer system;
(2) Ease of operation, i.e. flexibility of the equipment and supplier
in modifying the system to accommodate customer needs, variety of ways
to manipulate images, ease of routing images, and overall ease of
operation; (3) Service/Maintenance provide documentation of performance
uptime standards, response time, experience of service engineers and
quality assurance plan; and (4) Price. Offers must show the following:
the solicitation number; the date specified, name, address, telephone
number, fax number; a technical description of the items being offered
in sufficient detail to evaluate compliance with the requirements in
the solicitation. This may include product literature, or other
documents, if necessary; a unit price, a total price and any discount
terms for each time period, ''Remit to'' address, and a completed copy
of the provision at 52.212-3, Offers Representations and
Certifications-Commercial Items, with its proposal. Please submit
