synopsis/solicitation for commercial items prepared in accordance with
the format in FAR Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; Invitation For Bids (IFB) are being requested and
a written solicitation will not be issued. Solicitation document and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular 34. The SIC Code is 7699 and the small business
size is $5.0 million. Contractor shall provide on site maintenance
services at the VA Medical Center Cincinnati, Ohio, for a basic year
period commencing October 1, 1996 through September 30, 1997 with four
one-year pre-priced option years. Services are required for the
General Electric X-Ray System as follows: Item 0001 - MPX Generator,
RFX Table, XJ Hanger, L300 Image System and all other system items
including NEC VC9507; Item 0002 - ADVANTX RFX 90 with 8835 spot film
and all items under system ID# 513861RM248. Services shall include
on-call maintenance and two preventive maintenance inspection (PMI)
each calendar year on all equipment identified herein. Maintenance and
PMI procedures shall include but not be limited to: inspecting,
cleaning, lubricating, adjusting, calibrating, vacuuming, repairing,
replacing all component parts and glassware as needed, and any other
maintenance recommended by equipment manufacturer. Contractor shall
correct, without delay all FDA identified device hazards on equipment
covered herein. Contractor shall provide all necessary labor,
equipment, tools, supplies, materials, parts, technical manuals,
supervision, transportation and lodging to perform contract services.
All parts provided shall be new, current manufacture parts, compatible
with existing equipment. Contractor shall be available to perform
services during the work hours of 8:00 a.m. to 4:30 p.m. local time,
Monday through Friday, excluding federal holidays. Contractor shall
arrive on site to perform services within four (4) work hours after
notification and proceed progressively to complete services without
undue delay. Contractor shall maintain an adequate supply of the most
commonly used repair parts and have satisfactory sources of supply for
all other parts needed. Test equipment used shall be maintained in
proper calibration as recommended by equipment manufacturer, and
calibration shall be traceable to a national standard. Prior to
contract start date, Contractor shall provide a copy of the PMI
checklist identifying the PMI procedures to be performed. After each
PMI and service call, Contractor shall provide the Contracting
Officer's Technical Representative (COTR) with a completed service work
ticket. The billing cycle for this contract shall be monthly. Periods
less than a complete month shall be prorated. The following provisions
apply to this acquisition: 52.212-1 Instructions to
Offerors--Commercial Items; 52.212-2 Evaluation--Commercial Items;
52.212-3 Offer Representations and Certifications--Commercial Items;
52.212-4 Contract Terms and Conditions--Commercial Items; 52.212-5
Contract Terms and Conditions to Implement Statutes or Executive
Orders--Commercial Items, including clauses incorporated by reference
under Section (b)(1) through (11) and (15), and Section (c)(1) through
(3); 52.217-8 Option to Extend Services; and 52.217-9 Option to Extend
the Term of the Contract with one day written notice given and contract
duration not exceeding 5 years, 6 months. Period for acceptance of bids
is 60 calendar days. Bidders responding to this announcement shall
submit their monthly maintenance price for each item for the basic year
period and four one-year option periods, along with the information
required in 52.212-1, and a completed copy of provision 52.212-3 by
September 23, 1996, 2:00 p.m. local time. Services beyond September 30,
1997, 98, 99, 2000 and 2001 are contingent upon the availability of
funds. A site visit will be held on September 16, 1996 at 2:00 p.m.
