synopsis/solicitation for a commercial item prepared in accordance with
the format in FAR subpart 12.6 as supplemented with additional
information included in this notice. This announcement constitutes a
formal Request for Quotation (RFQ 500-65-96)and a written solicitation
will not be issued.The Standard Industrial Classification (SIC) Code
for this acquisition is 3841. Prices should be submitted as follows:
Item #1. Furnish, install & test two (2) double wall prefabricated
audiometric examination booths suitable for use in testing, calibration
& recording of aural acuity. These booths will be separate & share no
common walls. Outside dimensions of the booths shall fit into a space
not to exceed 9'8'' x 9'0''. Contractor shall be required to field
verify.Each will have an observation window to a control area.
Audiometric booths shall be constructed of modular,prefabricated panels
& assembly hardware. Enclosures must be capable of being
disassembled,moved & reassembled at some future date with minimal loss
of material or acoustical integrity. Audiometric booths shall be
furnished as completely prewired,upon assembly and ducted for HVAC
services. Electrical service shall be wired so as to allow for single
point electrical tie-in for each booth by the VAMC Project Engineer.
Booths shall be constructed of fire-rated components to obviate the
need for a fire sprinkler system & associated roof panel penetration
necessary for installation. Audiometric booths & all components shall
conform to the requirements of VAMC Audiometric Examination Booth
Specifications IB-11-87 dated June 1993 & VA Spec 1438b. All parts of
the booth having the same manufacturer's part number shall be
completely interchangeable with respect to installation & performance.
Booths shall consist of the following components as a minimum:
Standard Vibration Isolation System, Inner Room Acoustical Floor
Assembly, Wall & Roof Panel Assembly,Acoustical door units-36'' wide -
One booth shall have inswing, one booth shall have outswing, Standard
window units with double glazed, laminated,acoustical safety glass,
Assembly hardware, including connecting panel joints,Standard Recessed
electrical service with 12' power connection w/standard electrical
outlets,standard recessed fluorescent lighting with wall
switch,standard externally mounted outlet strip,standard wiring &
components,silenced forced air ventilation system,carpeting w/cove base
trim,paint & other specified finishes, dust seals/shields & closure
strips, standard jack panel,standard acoustical performance,
electromagnetic shielding,fire rating requirements. The contractor
shall be factory trained on proper booth installation techniques and
shall install audiometric booths and all specified components in VA
Medical Center. Contractor shall completely assemble audiometric booths
& all associated components which are provided by the manufacturer.
Outlets,plug strips,light fixtures and all other electrical components
internal to the booth shall be installed & booth wiring shall be
connected to the main junction box connection point. HVAC silencers
shall be installed on the audiometric booth walls or roof. Final
connection of HVAC services, main electrical power, thermostats and any
other building services is the responsibility of the VAMC.Contractor
shall furnish with each booth, two (2) copies of the equipment manual
containing installation,operation and maintenance instructions: a block
diagram showing wiring between related pieces of equipment which make
up the complete system and a parts data section. Upon completion of
installation, Contractor shall perform noise reduction tests in
accordance with ASTM E596-88 & submit test report results to the VAMC.
Test results shall include a complete description of the sample that
was tested, including an inventory of all enclosure components and
their weight. Noise reduction test data which does not include a
complete inventory of all components & their weight or has been
conducted in accordance with standards other than ASTM E596 shall not
be acceptable. Sound absorption tests shall be conducted in accordance
with ASTM C423-84a & shall have a penal weight equal to the average
panel weight of the panels used in the noise reduction tests.
Contractor shall provide a one year warranty for materials &
workmanship from the date of installation. Descriptive Literature (Ref
FAR 52.214-21) shall be submitted with quote or quote will be rejected
as non responsive. Offers shall be submitted FOB Destination. The
provisions at FAR 52.212-1, Instructions to Offerors--Commercial Items,
52.212-2, Evaluation-Commercial Items, 52.212-3 Offers Representations
and Certifications-Commercial Items (copy may be requested by fax),
52.212-4 Contract Terms and Conditions-Commercial Items, FAR 52.212-5,
52.222-26, 52.222-35, 52.222-36. 52.222-37. 52.225-3. 52.225-18 and
VAAR 852.219-70 apply to this solicitation. Offers are due by Close of
