DADA10) This is a COMBINATION SYNOPSIS/SOLICITATION for commercial
items prepared in accordance with the information in FAR subpart 12.6
and supplemented by additional information in this announcement and
comprises the solicitation in its entirety. This solicitation,
DADA10-96-R-0106, is issued as a Request for Proposal (RFP). PROPOSALS
ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED.
This solicitation document and clauses are those in effect through
Federal Acquisition Circular 90-39. This is an unrestricted
procurement. The Standard Industrial Code (SIC) is 2835, Size 500
Employees. DESCRIPTION: The contractor shall provide all testing
reagents, expendables, testing equipment and accessories required for
the identification of pathogenic organisms from specimens submitted for
diagnostic testing. All items listed are Abbott Laboratories,
Diagnostic Division of Abbott Park, IL and items supplied must be
Abbott Brand OR EQUAL. The following 15 catalog items are required as
follows, quantities are estimated per year for the base year and each
of the 4 option years: (1) Auszyme Monoclonal Test, #1980-24, 125 kits;
(2) HBsAg Confirmatory Kit, #1012-10, 60 kits; (3) Corzyme-M Test,
#1236-24, 66 kits; (4) AUSAB Kit #9006-24, 144 kits; (5) HAVAB-M Kit
(HAV-M), #9843-24, 60 kits; (6) Corzyme Kit (HB core-Total), #9977-24,
66 kits; (7) HBe Kit (HbeAg/Ab), #1235-24, 40 kits; (8) HAVAB
(HAV-Total), #7895-24, 90 kits; (9) Hepatitis C 2.0 Kit (HCV),
#4A14-24, 60 kits; (10) HIVAB-1 Kit (HIV-Ab), #3A11-24, 24 kits; (11)
Rotavirus Test Pack Kit, #6896-25, 12 kits; (12) RSV Test Pack Kit,
#2027-25, 30 kits; (13) Chlamydiazyme Kit, #1803-24, 8 kits; (14)
Rubazyme Kit, #7870-24, 40 kits; (15) HIV Blocking Assay, #7043-18, 50
kits. EQUIPMENT: The following equipment is to be provided by the
contractor on loan at no additional cost to the Government: 2 ea
Parallel Processing Centers (PPC), 2 ea Quantum II Analyzers, and 2 ea
Quick Wash Automatic Washers. The contractor will deliver, install and
maintain all equipment in accordance with manufacturer's
recommendations. ACCESSORIES to be included at no additional cost to
the government are: 12 each Waste/Water/Air Tubing, #6208-23; 12 bx
Reaction Trays (40X60), #7803-18; 8 pkg Reagent Blanking Beads,
#6208-42; 10 bx CMDT Dispense Tip Remote Assembly, #6208-31; 3 each 300
ul CMDR Dispenser, #6208-36; 3 each 5 ml Tricontinent OPD Dispenser,
#7186-11; 3 each 1 ml Tricontinent Dispenser, #7186-20; 12 each 100 ul
Tricontinent Dispenser, #6208-33; 12 each 200 ul Tricontinent
Dispenser, #6208-35; 12 each 400 ul Tricontinent Dispenser; #6208-38;
12 each 50 ul Tricontinent Dispenser, #6208-32; 2 each STD-EZE,
#6208-24; 6 each PPC Lamp, #6208-75. SALIENT CHARACTERISTICS OF TEST
SPECIFICATIONS: All items are required for the identification of
pathogenic organisms from specimens submitted for diagnostic testing.
All Assays must be FDA licensed and a participating member in the
College of American Pathologist (CAP) Interlaboratory Comparison
Program. DELIVERY for all items will be on an ''ON-CALL'' basis and
will be delivered to Brooke Army Medical Center, DPALS, Microbiology
Section, Bldg. 2630, Fort Sam Houston, Texas 78234. All items shall
have at least a six (6) month expiration date upon receipt. SALIENT
CHARACTERISTICS OF EQUIPMENT: Equipment must maintain sample integrity
from start to finish (positive ID) and is to feature walk away
automation, be equipped with a data management system that will provide
positive sample identification of patients, control the test system,
capture test date, produce a cartable patient test report, recall
sample history, track reactive samples, track quality control by run,
lot number, and interface with Composite Health Care Computer System
(CHCS). SERVICES REQUIRED: Vendor will service the assay test equipment
and the data management system within 24 hour notification of
breakdown; provide a telephone number for 24 hours technical support
and capability to respond telephonically within 30 minutes for a
request for technical assistance. For instruments requiring service; 4
hour phone response time and/or repair or provide replacement
equipment within 24 hours after notification. Provide maintenance,
calibration, instrument and software upgrades and assistance in
validation and retain title to all instrumentation and ancillary
equipment. Provide training necessary for the operation of the system
at no cost to the government. If training is provided at vendor's
facility, vendor is responsible for all costs incurred (i.e., food,
lodging and transportation). Provision 52.212-1 INSTRUCTIONS TO
OFFERORS-COMMERCIAL ITEMS applies. Submission of Offers: Proposals may
be submitted on letterhead stationery and as a minimum must show the
following: Solicitation number, time specified in the solicitation for
receipt of proposals; name, address, telephone number, fax number; a
technical description of the items being offered in sufficient detail
to evaluate compliance with the requirements of the solicitation.
Offers to include product literature, or other documents if necessary;
terms of any express warranty; unit price, total price and any
discount terms; a ''Remit to'' address, if different than the mailing
address; a completed copy of the representations and certifications at
FAR 52.212-3; acknowledgement of Solicitation Amendments; include a
statement specifying the extent of agreement with all terms,
conditions, and provisions included in the solicitation. Offers that
fail to furnish required representations or information,or reject the
terms and conditions of the solicitation may be excluded from
consideration. The offeror agrees to hold the prices in its offer firm
for 120 days. Offers not agreeing to 120 days for government
acceptance after the date offers are due will not be considered and
will be rejected. Contract Award: The government intends to evaluate
offers and award a contract without discussion with offerors.
Therefore, the offeror's initial offer should contain the offeror's
best terms from a price and technical standpoint. However, the
government reserves the right to conduct discussions if later
determined by the Contracting Officer to be necessary. The government
may reject any or all offers if such action is in the public interest,
accept other than the lowest offer; and waive informalities and minor
irregularities in offers received. Provision 52.212-2
EVALUATION-COMMERCIAL ITEMS applies. Award resulting from this
solicitation will be to the responsible offeror whose offer conforming
to the solicitation will be most advantageous to the government, price
and other factors considered. The following factors shall be used to
evaluate offers: (a) Compliance with the salient characteristics, (b)
past performance, and (c) price. Non-price evaluation factors (taken as
a whole) are significantly more important than cost or price. To be
determined technically acceptable, the offeror must furnish product
literature that demonstrates the products offered, meet each and every
requirement stated in the description given above. The Government will
evaluate offers for award by adding the total price for all options to
the total price for the basic requirement. A written notice of award
or acceptance of an offer, mailed or otherwise furnished to the
successful offeror within the time for acceptance specified in the
offer, shall result in a binding contract without further action by
either party. Before the offer's specified expiration time, the
Government may accept an offer (or part of an offer), whether or not
there are negotiations after its receipt, unless a written notice of
withdrawal is received before award. Provision 52.212-3 OFFEROR
REPRESENTATIONS AND CERTIFICATIONS-COMMERCIAL ITEMS applies. Offerors
are to include a completed copy of the ''Representations and
Certifications'' at FAR 52.212-3 with their offer. Clause 52.212-4
CONTRACT TERMS AND CONDITIONS-COMMERCIAL ITEMS applies. Clause 52.212-5
CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES OR
EXECUTIVE ORDERS-COMMERCIAL ITEMS applies. Under sub-paragraph (b) in
52.212-5 the following clauses apply: 52.203-6 w/Alt I, 52.203-10,

252.212-7001 CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT
STATUTES OR EXECUTIVE ORDERS APPLICABLE TO ACQUISITIONS OF COMMERCIAL
ITEMS applies. Under sub-paragraph (b) in 252.212-7001 the following
DFARS clauses apply: 252.205-7000, 252.219-7003, 252.219-7005,
252.225-7001 and 252.233-7000. The following additional DFARS provision
is incorporated by reference: 252.225-7000(offerors are to complete and
return the certificate with their offer). This is a firm fixed price
contract. Contract period is from effective date to 30 SEP 97 plus 4
one year options. Proposal shall be mailed or hand carried to Central
Contracting, MEDCOM, ATTN: MCAA-CB, Building 2015, 1105 Beebe Loop,
Fort Sam Houston, Texas 78234-6000 by 3:00 PM, within 15 calendar days
