65 -- MAJOR SURGICAL TABLE SOL RFQ 614-83-96 DUE 091896 POC Jerry Shepherd, Contract Specialist Phone No. 901-577-7228 This is a combined synopsis for a commerical item prepared in accordance with the subparts in 12.6 as supplemented with additional information included in this notice. This announcement constitutes a formal Request for Quotations (RFQ #614-83-96), and a written solicitation will NOT be issued. The solicitation document and incorporated clauses and provisions are those in effect through Federal Acquisition Circular 90-41. The Standard Industrial Classification (SIC) code for this acquisition is 5047. Prices shall be submitted as follows for each item: 1. Brand name or equal Quantum 3080SP, Major Surgical Table. Table shall be battery powered, 120V; have a manual foot-operated pump for power back up; industry exclusive side tilt cylinder with mechanical lock, fluid tight, shock resistant hand control programmable for normal or reverse patient orientation; single command return-to-level capability; four conductive casters and three self-compensating floor locks; manual kidney elevator; completely radiolucent tabletop shall be at least 75''L X 20''W; have height adjustment from 27'' - 44''; have a 400 lbs. patient weight capacity; be certified by CSA; ETL listed; shall include 2'' DAD conductive pad, four section I/A compatible tabletop and X-ray top. 2. IV ARMBOARD with 3'' DAD PAD: Supports arm at a comfortable angle. Locks by gravity to table siderail. adjusts 180 degrees horizontally. Supplied with a 15'' Velcro are restraint strap. 3. CONDUCTIVE RESTRAINT STRAP: Designed with quick-release buckles on both ends for easy, positive adjustment, wide section measures 6'', length adjusts to fit all body types. 3. CLARK SOCKETS (Pair): Used for mounting accessories anywhere along table siderail. Offers shall be submitted with FOB Destination pricing and shall be delivered and installed within 30 calendar days after contract award along with 2 user's/operator's manuals and 2 service manuals with schematics and parts list at NO COST to the Government. The provision at FAR 52.212-1 Instructions to Offerors -- Commercial items, applies to this solicitation. All offerors are to include, with their offer a completed copy of provision 52.212-3 Offeror Representations and Certifications - Commercial items ( a copy of this provision may be requested by FAX to 901-577-7242). The following FAR clauses apply to this acquisition: 52.212-4 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items; as well as FAR clauses sited in FAR 52.212-5; 52.222-26; 52.222-35; 52.222-36; 52.222-37; 52.225-3; 52.225-18; VAAR 852.219-70 is added by addendum. Offers shall be due by close of business September 18, 1996 all offers shall be signed by an official authorized to bind the offeror's organization.
