(215)697-9696. This is a combined synopsis/solicitation for commercial
items prepared in accordance with the format in Subpart 12.6 as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation, proposals are being
requested and a written solicitation will not be issued. Solicitation
number N00140-97-R-N180 applies and is issued as a Request for
Proposal. The FISC Norfolk Det. Philadelphia is soliciting on an
unrestricted basis, all responsible sources may submit a proposal. The
requirement is for a firm fixed-price contract for the following line
items to be provided for one base year and three twelve-month option
periods. Lot I is the Base Period which spans from 01 Oct 96 through 30
Sept 97 for Line Item 0001, and spans 01 Apr 97 through 30 Sept 97 for
Line Item 0002. Note: maintenance not to begin on Item 0002 until 01
Apr 97 as it under manufacturer's warranty until that date. Line Item
0001 is for the maintenance of a Seimens Multispect 2 Nuclear Medicine
Imagining Machine, quantity: 12, unit: month. Line Item 0002 is for
the maintenance of a Seimens Multix Polydoros X-ray Machine, quantity:
6, unit: month. Lot II - First Option Year - 01 Oct 97 through 30 Sept
98 includes Line Item 0003 and 0004. Item 0003 is for the maintenance
of a Seimens Multispect 2 Nuclear Medicine Imagining Machine, quantity:
12, unit: month. Item 0004 is for the maintenance of a Seimens Multix
Polydoros X-ray machine, quantity: 12, unit: month. Lot III - Second
Option Year - 01 Oct 98 through 30 Sept 99 includes Line Item 0005 and
Line Item 0006. Item 0005 is identical to Item 0003. Item 0006 is
identical to Item 0004. Lot IV - Third Year Option - 01 Oct 99 through
30 Sept 00 includes Line Item 0007 and Line Item 0008. Line Item 0007
is identical to Item 0003. Line Item 0008 is identical to Item 0004.
Maintenance is to be rendered at the machines' current locations.
Service under Items 0001, 0003, 0005 and 0007 is to be performed at the
Naval Hospital, Great Lakes, IL in the Nuclear Medicine Department. The
following parts will be maintained under all odd numbered Line Items:
Multispect 2 System, Icon P Computer, Macicon, Helios Laser Imager,
Video Color Printer Seiko -S698 Kit, Modem Remote Access Package and
the Optical Disk. Maintenance under Items 0002, 0004, 0006 and 0008 is
to be performed at the Branch Medical Clinic, Bldg 237, Great Lakes,
IL. Glassware will be maintained under all even numbered Line Items.
The Contractor's Service Person shall check in with the Biomedical
Repair Division before the performance of any Contract Specification.
The Biomedical Repair Division is located in the basement of Bldg 200H
of the Naval Hospital, Great Lakes, IL. The Service Person shall also
check out with the Medical Repair Division upon completion of service.
He/she shall furnish the Medical Repair Division with a copy of a
service report which includes the following information: a) identifying
data - especially the Hospital barcode numbers. b) description of
malfunction upon arrival. c) all service performed d) all parts
replaced/ordered. e) follow-up information (parts on order, expected
completion date, status of operation, etc.). The Contractor's Service
Person shall report, in writing all discrepancies noted which are
suspect of operator error, abuse or neglect. He/she shall receive
advance approval prior to performing any services which are not covered
under existing contract specifications. All service and preventative
maintenance shall be performed during normal working hours of 8:00 am
- 5:00 pm Monday - Friday. The equipment shall receive two (2)
preventative maintenance inspections during each period of performance.
The inspections shall be done in the months of February and August.
Each preventative maintenance inspection shall include the following as
a minimum: a) visual inspection of each unit b) check operation of all
controls, lights, and indicators and calibrate as necessary. c) check
condition of all cable and hose connections and perform corrective
action as necessary. d) lubricate as necessary. Corrective maintenance
shall include the following as a minimum: a) written determination of
the cause of the problem. b) repair or replace and calibrate equipment
as authorized by the Naval Hospital, Great Lakes, IL. c) return each
piece of equipment to fully operational status. Unless otherwise
specified by special specification, on-site response time shall be
within 24 hours after initial trouble call was answered. Telephone
response time should be same day, but no longer than 24 hours after
trouble call is placed. Replacement Parts shall be furnished in
exchange for the parts being replaced. Component parts, assemblies or
subassemblies shall be replaced with new or refurbished items at the
contractors option. Refurbished items shall carry the same warranty, if
any, applicable to similar new components. Services shall be invoiced
monthly. This solicitation and incorporated provisions and clauses are
those in effect through Federal Acquisition Circular 90-37. The
following FAR provisions and clauses apply to this solicitation and are

Contract Terms and Conditions Required to Implement Statutes or
Executive Orders - Commercial Items. As an addendum to 52.212-5
paragraph (b), the following FAR clauses are incorporated by reference:
52.203-6 Restrictions on Subcontractor Sales to the Government (Oct
95), 52.203-10 Price or Fee Adjustment for Illegal or Improper Activity
(Sept 90), 52.219-8 Utilization of Small, Small Disadvantaged and
Women-Owned Small Business Concerns (Oct 95), 52.222-35 Affirmative
Action for Special Disabled and Vietnam Era Veterans (Apr 84),
52.222-36 Affirmative Action for Handicapped Workers (Apr 84) and
52.222-37 Employment Reports for Special Disabled Veterans and Veterans
in the Vietnam Era (Jun88). The following DFAR provisions and clauses
apply to this solicitation and are incorporated by reference: DFAR
252.212-7000 Offeror Representation and Certification - Commercial
Items and DFAR 252.212-7001 Contracting Terms and Conditions Required
to Implement Status Applications to Defense Acquisitions of Commercial
Items. Award will be made to the low price, responsible offeror whose
offer conforms to the solicitation. The Government reserves the right
under FAR clause 52.215-34, ''Evaluation of Offers for Multiple
Awards'', to award to more than one source under this solicitation.
Proposals in response to the solicitation must be submitted in
accordance with the instructions set forth in FAR 52.212-1 paragraph
(b). As a minimum, proposals shall include a price proposal on company
letterhead or the SF1449 for the requested line items showing unit
price and extended price. If the SF 1449 is not used, proposals must be
provided on company letterhead and include a statement specifying the
extent of agreement with all terms, conditions and provisions of the
solicitation. Proposals must also include technical information,
remittance address, completed copies of FAR 52.212-3 and DFAR
252.212-7000 and any warranty, price or discount terms. Copies of
clauses 52.212 and 252.212-7000 will be provided upon offeror request.
Responses to this solicitation are due no later than 4:00 pm, EST, 3
Sept 96. All offers shall be sent to FISC Norfolk Det. Philadelphia,
