source basis with the intent to contract with Coherent, Inc. for a
Coherent Ultrpulse 5000L, 208 volt CO2 Surgical Laser System, based on
market survey results demonstrating that no other product meets the
required specifications. This is a combined synopsis/solicitation for
commercial items prepared in accordance with the format in FAR Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; proposals
are being requested and a written solicitation will not be issued.
Solicitation number N00244-96-R-5157 applies and is issued as a request
for proposal (RFP). This combined synopsis/solicitation and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular 90-38 and Defense Acquisition Circular 91-10. The
standard industrial classification code is 3845 and the business size
standard is 500 employees. This requirement is for a fixed price
contract for four line items as follows. Item 0001: quantity 1 system;
Coherent Ultrapulse 5000L, 208 volt CO2 surgical laser system
including short duration, high energy pulsed delivery of between 1-200
millijoules per pulse; 1-80 watts average power in the pulsed or CW
mode; milliwatt capability; 5 mw red and 2.5 mw yellow HeNe aiming
beam; compact tankless sealed laser system - RF excited; Duralite
articulating arm with permanently aligned optics; L.E.D. control panel;
XA218 (0.2mm) and XA219 (1.0mm) spot size handpieces. 0001AA: quantity
1 case (6); CO2 goggles. 0001AB: quantity 1 each; warning sign.
0001AC: quantity 1 each; operator's manual. 0001AD: quantity 1 set;
lens cleaning accessories. Items 0001AA-0001AD shall be listed at no
additional cost (Not Separately Priced). Item 0002: quantity 1 system;
Clearspot 350 micromanipulator for CO2 surgical systems - provides
precise microsurgical cutting, ablation, and coagulation with spot
sizes as small as 0.35mm; can adapt to Zeiss (incl wide aperture
microscopes, Wild Vasconellos, and Storz microscopes); provides clear,
unobtrusive view with no struts or linkages to get in the way; high
clarity hot mirror for maximum light transmission with no discoloration
of tissue; maximum operating flexibility with 5 working distances of
200mm, 250mm, 300mm, 350mm, and 400mm; adjustable focal distance right
on the Clearspot 350 with no need to change micromanipulators and it
can be done in seconds; zero parallex design which delivers treatment
beam along visual axis; Diffractive Optical Technology to achieve a
crisper, cleaner aiming beam that is in perfect match with CO2 beam.
Item 0003: quantity 1 each; Coherent/Nezhat laparoscopic coupler which
serves as a direct link between the laser and laparoscope, with no
requirement for adjustment or alignment by the surgeon. The following
item shall be listed at no additional cost (Not Separately Priced).
Item 0004: quantity 8 hours; in-hospital training for staff by
contractor clinical educator(s); qualifies for up to 4.0 contact hours
by ANA. Delivery of items 0001-0003 shall be within 30 days of
contract award under FOB Destination terms to San Diego, CA. Item 0004
delivery terms will be stated in the contract. Training will cover
operational and functional procedures for the equipment. Inspection and
acceptance will be at destination. FAR provision 52.212-1, Instructions
to Offerors--Commercial Items, is hereby incorporated by reference,
including clause 52.211-6, Listing of Used or Reconditioned Material,
Residual Inventory and Former Government Surplus Property. All offerors
are required to submit completed copies of the following FAR and DFARS
provisions: 52.212-3, Offeror Representations and
Certifications--Commercial Items; 252.212-7000, Offeror Representations
and Certifications--Commercial Items, including 252.225- 7035, Buy
American Act--North American Free Trade Agreement Implementation
Act--Balance of Payments Program Certificate, by addendum. FAR clause
52.212-4, Contract Terms and Conditions--Commercial Items, applies, as
well as the following addendum clauses: 52.211-5, New Material;
52.211-7, Other than New Material, Residual Inventory & Former
Government Surplus Property; 52.215-43, Audit--Commercial Items; and

Contract Terms and Conditions Required to Implement Statutes or
Executive Orders Applicable to Defense Acquisitions of Commercial
Items, applies with the following clauses applicable for paragraph (b):
252.225-7012, Preference for Certain Domestic Commodities;
252.225-7036, North American Free Trade Agreement Implementation Act;
252.233-7000, Certification of Claims and Requests for Adjustment or
Relief; 252.247-7024, Notification of Transportation of Supplies by
Sea. The government intends to make a single award to the responsible
offeror whose offer is the most advantageous to the government. FAR
provision 52.212-2, Evaluation of Commercial Items, applies with
paragraph (a) completed as follows: Award will be made to the offeror
that meets the synopsis/solicitation's minimum criteria for technical
acceptability at the lowest price. To be determined technically
acceptable, the offeror must furnish product literature that
demonstrates the products offered meet all requirements stated in the
above purchase descriptions for Items 0001 through 0004. Sealed offers
(original and 1 copy) must be received no later than 3:00 PM (PT) on
August 26, 1996. Offers sent via the US Postal Service should be mailed
to the Fleet and Industrial Supply Center, Bid Officer Code 210A94, 937
N. Harbor Dr., San Diego, CA 92132-0212. All offers not sent through
the US Mail will be considered to be hand carried and direct delivered.
Hand carried offers must be delivered directly to and placed in the
depository located in Building 1, 2nd Floor Bid Desk, Room 207, 937 N.
Harbor Dr., FISC Regional Contracts Department. Direct delivery of
offers is only possible during weekdays, excluding federal holidays,
