The Alaska Area Native Health Service will be issuing a Request for Quotation on or about July 1, 1996, to provide a Community Health Representative (CHR) Basic Training Course. This proposed acquisition is less than $50,000 and any award issued will be made under the Simplified Acquisition procedures. Upon award the contractor shall provide an accredited CHR Basic Training course that is modified to provide a two week on-site (local) training component and a final interactive week at a centralized site. Contractor shall: solicit applicants; provide course training materials; provide all administrative support and materials; provide tutorial assistance; conduct the at-home and on-site portions of the course; coordinate and administer the travel arrangements and accommodations for the student participants to the Black Hills Training Center, Rapid City, SD; and at the completion of the course contract shall provide a final written report.
