Architect or architect/engineer firm to provide services for advanced
planning and design of a project that may include, but is not limited
to: Ambulatory Care Addition. At a minimum, this project proposes to
construct a new two-story addition (46,000nsf) to connect physically,
to the existing Main Hospital Building #111. The new construction will
house ambulatory care, a mental health clinic, ambulatory surgery and
radiology, cardiology and pulmonary medicine. Also in the scope of
work is the renovation of 10,000 nsf of existing ambulatory care space
in existing Building #111. Asbestos removal will be required in the
first floor of Building #111. Site work will be minor and include
modifications to the front entrance for the ambulance and driveway.
Required services will include schematics and/or design development,
including all appropriate architectural, interior design and
engineering disciplines and a firm estimate of construction cost. The
contract will include a government option for construction documents
preparation and construction period services. Only firms with in-house
architectural services, experienced in the design of similar type
projects, including the use of the sustainable design, will be
considered. Emphasis will be placed on the estimating capabilities of
the applicants. Applicants must have an established working office in
the state of Wisconsin. This proposed acquisition is a 100% small
business set-aside. Firms must indicate in response whether they meet
the requirements for a small business concern. No firm will be
considered unless an application consisting of a completed SF255 and
such other evidence of experience and capabilities the applicant
considers relevant, is received by the VA A/E Evaluation & Program
Support Service within 30 days of the date of this advertisement. A
current SF254, representing and differentiating between the personnel,
experience and capabilities of all involved offices, must be included
with the application together with current SF254s for each proposed
consultant's office involved. Applications will be rated based on the
relevant experience of both the firm(s) and assigned individuals, the
applicant's capacity to do the work, record of past performance on VA
work, geographic consideration, and participation of minority and
women-owned consultants. Award of a contract is dependent upon the
availability of funds. This is not a request for a proposal. Note: the
''covenant against contingent fees'' clause is applicable to this
