H -- STATEMENT OF CONDITIONS SOL RFP 556-19-96 DUE 082796 POC Contracting Officer, Pamela Smith, (847) 578-3722 Contractor shall provide fire protection support in the form of an assessment of identified buildings to determine if any deficiencies exist, in accordance with the criteria as established in the Life Safety Code and completing the ''Statement of Conditions'' (SOC) as described in the 1966 Joint Commission of Accreditation of Healthcare Organization Standards (JCAHO) for the VA Medical Center, North Chicago, IL, and clinics located in Rockford, Gurnee and Evanston, IL. Completion time for this project is 45 calendar days from receipt of notice to proceed. Offerors will be evaluated on the following factors listed in order of importance: (1) Provide listing of facilities, facility owners/operators and dates of completed JCAHO SOC's that have been accepted by JCAHO; (2) Cost.
