Q -- OPHTHALMOLOGY SERVICES SOL 653-11-96 DUE 081496 POC Contracting Officer, Kathy Davis, (541) 440-1000x4251 The VA Medical Center, Roseburg, Oregon proposes to acquire Ophthalmology Services, to be performed at the contractor's facility which should be geographically located in close proximetry to the Medical Center to meet agency needs. Services will include clinics to perform necessary consultative evaluations and treatment to VA Beneficiaries. Contractor physician will be board certified or board eligible and shall meet the requirements for credentialing and privileging specified by Medical Center Bylaws. Period of performance will include base year plus two additional one year options to extend, beginning approximately September 1, 1996 through August 31, 1997. All requests for RFP must be in writing to the contracting officer or by fax 541/440-1276 and must include name, address, phone number of requestor, along with the solicitation number and reference to Ophthalmology.
