608-262-7068. Provide professional architectural and engineering
services for project design, preparation of construction/bid documents
and construction period services for VA project 607-382, Expand
Ambulatory Care. Work involves, but is not limited to, designing for
the construction of an addition to the D-wing of building No. 1 at the
William S. Middleton Memorial Veterans Hospital in Madison, Wisconsin.
This work includes providing ambulatory care space at what will become
the wing's sixth floor. The space shall consist of but not be limited
to; Three amublatory clinic ''Pods'' consisting of eight
exam/treatment rooms, one conference room and one consultation room per
''Pod'': A patient waiting area complete with handicap accessible
toilet facilities, a reception area, a vitals area, a nurse station, a
housekeeping aids closet, mechanical space, electrical space, soiled
linen room(s), clean linen room(s), storage areas, relocation of a
mechanical penthouse to the extent possible as determined by the
design, Relocation of duct shafts from fourth and fifth and floor as
required by design and redesign of D-wing lobby and toilet areas to
accommodate entrance design to the space. Extension of Data, Fire
alarm, PA, communications systems shall be included. The design shall
accomodate any required changes to the D-wing sixth floor elevator
lobby and A-wing space as dictated by design and present layout
problems. The mechanical/electrical penthouse and all associated
functions shall be relocated from the present roof to the new addition
roof. New utilities may be enclosed and located within a redesigned
penthouse or at other areas as dictated by the design. Access to the
new roof shall be provided from the D-wing similar to the present roof
access door. The structure and utilities shall be designed for future
vertical expansion just as the existing addition constructed in
1995/1996 has been done. Existing electrical and mechanical utilities
have been designed for vertical expansion. Chilled water pipe size
feeder and return lines shall be evaluated and replaced as required.
(Expected problems based on data from 1995/1996 expansion). Verify and
Reuse connection locations to existing utilities when feasable.
Generic CADD Floor Plans in Autocadd R.12 can be provided by the VA as
well as specifications in word perfect 5.1 for windows which shall be
edited and prepared by the A/E. As-built drawings (if any) shall be
reviewed at the VA Engineering offices unless approved otherwise by
special arrangement. Hardcopy, as well as computer (word perfect 5.1
for windows) and cadd (Autocadd R.12 or R.13) deliverables shall be
required at various design stages in accordance with the A/E's
negotiated contract. Prospective offerors shall provide evidence of
accurate estimating in the form of a comparison between their estimates
and actual project amounts. The area of consideration is limited to a
250 mile radius of Madison, Wisconsin. Interested Firms shall submit
Standard Forms 254 and 255 no later than May 6, 1996. Estimated
construction cost is 1-5 million dollars. SIC is 8712. Small business
size standard is 2.5 million. Contract award shall be subject to the
