this notice. This announcement constitutes the only solicitation.
Offers are requested. A written solicitation will not be issued. In
responding to this invitation to bid, offerors must refer to
solicitation #DTCG39-96-B-3KB907. This solicitation incorporates the
provisions and clauses in effect through Federal Acquisition Circular
90-37. The SIC code for the services to be acquired is 7363. The small

Nursing Personnel to perform Registered Nursing Services at the U. S.
Coast Guard Academy (CGA), New London, CT. Perform services in
accordance with the American Registered Nurse Association standard of
practice, COMDTINST M6000.1B, Coast Guard Medical Manual and COMDTINST
1850.1, Veterans Administration Guide for Disability Examinations.

PERSONNEL QUALIFICATIONS - Provide Nursing Personnel who meet the
following criteria: 1) Possess current state of Connecticut
license/registration/ certificate as applicable and appropriate for the
services provided. 2) Possess CPR certification. 3) Complete annual
in-service training as follows: a) Full time employee-a minimum of
twelve hours per year. b) Part time employee-pro-rated based upon the
number of hours worked, but not be less than four hours annually per
employee. HEALTH QUALIFICATIONS-Prior to starting work, provide the
Contracting Officer with a certification that all Nursing Personnel
working on this contract: a) Are free from mental or physical
impairments which would restrict them from providing the services
described herein or endanger the health and well being of patients. b)
Received at least three doses of recombinant hepatitis B vaccine
currently licensed in the United States, received an initial dose of
hepatitis B vaccine (the series shall be completed within 6 months of
the employee start date), declined hepatitis B vaccine as set forth in
OSHA guidelines, or provided evidence of immunologically effective
anti-HB levels in lieu of proof of vaccination. c) Show immunity to
Measles, Mumps, and Rubella; varicella immune status; and a current PPD
reading or evaluation. d) Received a physical examination and the date
of the exam and the name of the physician who performed the
examination are identified. QUALITY ASSURANCE/RISK MANAGEMENT
MONITORING-The Contractor shall comply with the Quality Assurance
Program in accordance with the M6000.1B, Chapter 13. Services shall
meet or exceed the standard of practice prescribed by the American
Registered Nurse Association. The CGA will monitor the contractor's
performance using any or all of the following methods: Examination of
patients, Review of the appointment logs, Review of the Clinic Data
Collection Sheets, Review of Outpatient Records, Review of Patient
Satisfaction Surveys, Review of at least 5% of health record entries
made by the Nursing Personnel and other methods as appropriate.
TASKS-Perform a full range of professional nursing concepts and
practices using Government furnished facilities, equipment and supplies
to a variety of patients admitted to the inpatient treatment area and
the outpatient treatment area. Provide clinical services including but
not be limited to: 1) Assess patients needs for nursing care, plan for
nursing interventions, assess the effectiveness of the interventions
and alter the nursing plan and interventions appropriately. 2) Monitor
and assess patients for changes in condition, notify physicians,
dentists, or caregivers appropriately and initiate interventions to
provide comfort, to prevent distress or further injury, to protect the
patient from injury or further deterioration to their condition. 3)
Administer oxygen, IV fluids, and prescribed oral, subcutaneous and
intramuscular medications. 4) Manage nasal-gastric and other drainage
tubes and suction tubes. 5) Assist in instructing Coast Guard Health
Services Technicians, LPNs/LVN's, nursing assistants and ancillary
personnel. 6) Prepare patients for surgical and/or diagnostic
procedures. 7) Observe postoperative and other patients requiring close
observation as well as those under therapy for adverse conditions or
reactions. 8) Prepare, operate, and monitor specialized equipment such
as cardiac monitors, defibrillators, etc. 9) Inventory medicine and
drug stocks to ensure adequate supplies and controls. 10) Treat
patients who present a Coast Guard Health Record, a clinical record or,
in the case of active duty members and/or DEERS eligible beneficiaries
whose records are lost/misplaced, those patients that the Medical
Administrator or his/her designee approves. Refer any patient needing
care without a complete clinical record to the Clinic's Medical
Administrator or his/her designee. 11) Maintain clinical records. Each
record contains the following information: Patient Name, Rank/rating
or relation to active duty member, Active duty member's social security
number, Active duty member's unit address or non-active duty patient's
home address. 12) Ensure entries are adequate for continued proper
care of the patient, to evaluate quality of care, to substantiate
disability, if indicated, and to justify claims for services presented
to the Coast Guard. 13) Annotate all entries in the ''SOAP'' method,
as follows: a) Date, time and place of service; b) S: Subjective, cc:
(Chief Complaint) sore throat, cough, etc., hpi: (History of Present
Illness) onset of symptoms, all problems, review of symptoms, pmh:
(Past Medical History) any related problems in the past that may be
present with chief complaint, fh: (Family History) any diseases,
chronic/acute, possibly related to present complaint, all: (Allergies)
any known allergies to drugs/medications, etc., c) O: Objective-First
visual assessment/evaluation of the patient's general appearance:
Limping, bleeding, double over, etc., pe: All results of physical exam,
vital signs, lab, x-ray, and any other results, d) A: Assessment, imp:
Impression/diagnosis, includes rule out, e) P: Plan. List of
medications prescribed, lab, x-ray, physical therapy treatment plans,
special studies, duty status recommendation, return appointments,
referrals, etc. f) Contractor's signature, and g) Contractor's stamped,
typewritten, or printed name and degree. HOURS OF WORK-Provide
Registered Nursing services on a routine basis, full time, up to
fifty-six (56) hours per week at the CGA, New London, CT. Normally,
provide services from 7:00 AM to 12:00 PM and from 1:00 PM to 4:00 PM
exclusive of Federal holidays. In the event that the Superintendent of
the CGA closes the base and/or the Clinic, the contractor's services
will be deemed to be not necessary, and the contractor will not be paid
for the period of closure under this contract. The CGA will attempt to
request Nursing Personnel at least twenty four hours prior to
reporting time in order to assure prompt arrival of Nursing Personnel.
Emergency requests may be placed at any time by the CGA. All
information regarding reporting time and assignment will be provided at
the time of the initial call. The following provisions and clauses will

shall indemnify and hold harmless the CGA from any liability producing
act or omission by the contractor, its employees and agents occurring
during contract performance. The contractor shall maintain medical
liability insurance, in a coverage amount acceptable to the contracting
officer, which is not less than the amount normally prevailing within
the local community for Registered Nurses. The contractor and all
Nursing Personnel shall maintain confidentiality of all patient and
related information. The applicable Service contract Act Wage
Determination is DoL Wage Determination No. 94-2089 Rev No.5 Dated
06/07/95. Funds are available for Item 0001 only. Items 0002 through
0005 are options which the CGA MAY chose to exercise unilaterally. CBD
