The Contractor shall provide Computerized Tomographic (CT) radiologic studies and interpretations for various Navajo Area Indian Health Service hospitals and health centers. Contractor shall provide CT scan studies in a contractor-owned CT unit to be located on-site at Tuba City Indian Medical Center, Tuba City, AZ. The Contractor shall provide all necessary personnel, equipment, and supplies required to deliver CT scan services. The Contractor's medical personnel shall be licensed, certified and/or registered with the appropriate Boards, Registry, and/or State of Arizona. Resultant contract will be a requirement-type contract to be effective for an initial twelve (12) month period with two (2) one-year option periods. All requests for solicitation must be in writing.
