(317)298-1180x1580 The U. S. Customs Service has a need for a
Contractor to provide Drug testing proficiency specimens (Blind
Samples) to be utilized in support of the Urinalysis Drug Screening
Quality Assurance Program. In order to qualify as a Contractor for this
service the laboratory must be certified by the Department of Health
and Human Services or be a part of a Department of Health and Human
Services recognized Certification program in accordance with Mandatory
Guidelines for Federal Workplace Drug Testing Programs. The
performance period is for a one year period from date of award with one
option year. Unit of issue is each. Samples will constitute a minimum
of 10 percent of all samples to a maximum of 250 per quarter or 1000
per year. All responsible sources may submit a Technical Proposal with
Quotation which will be considered. Award will be based upon Technical
Proposal weighted two times more than Price. See Numbered Note One. Fax
