Contractor to provide General Surgery services to inmates at the Federal Correctional Institution, Phoenix, Arizona, which is located approximately 15 miles north of Phoenix on I-17. Period of performance is from the date of award through 9/30/96, with four, one-year option periods. Contractor to provide one four-hour session per month (second Thursday of every month) which is approximately 12 sessions for the base period (estimated award date of 10/1/95) and 12 sessions for each option period.
