Q -- HOME OXYGEN SERVICES SOL IFB #533-7-96 DUE 081695 POC Contact Point, Bertha Geraghty, (914)838-5262, Contracting Officer, FAX 914-838-5181 The Department of Veterans Affairs Medical Center, Castle Point, NY has a requirement for home oxygen service to be provided to eligible routine and life support patients of the Medical Center. There are approximately 15-20 patients requiring this service each month. The contractor will be required to provide oxygen in cylinders, a backup oxygen source, oxygen concentrators, regulators, cylinder carts, nasal cannulas, humidifiers, tubing connectors, extension tubing, and training on the use of the equipment. In addition, the contractor will be required to refill empty cylinders as needed. This will be a requirements type of contract. The period of performance will be from October 1, 1995 through September 30, 1996. All requests for the solicitation must be in writing on company letterhead. Include in the request your name, address, telephone number and fax number, size of business, and the solicitation number. All responsible sources may submit bids which shall be considered by the agency. Requests for copies of the solicitation should be received no later that 15 days from the date this notice is published. Contractor must meet JCAHO requirements.
