Concerns or individuals meeting the above requirements are requiredto submit a capability statement demonstrating their ability to access this data to ProgramAcquisition Branch, 255 East Paces Ferry Road, NE, Room 405, Atlanta, GA 30305 within15 days of publication of the announcement citing RFP 200-95-2958(P). Offerors whorequest a copy of the solicitation and/or simply provide a statement that they can perform thework required will not be considered. In the absence of other qualified sources, it is theintent of CDC to make sole source award to the Minnesota Department of Health. TheStandard Industrial Code is 8733, Size Standard $5 million. The scheduled issue date isapproximately July 07, 1995. The contemplated period of performance is 15 months.
