Lancaster Rd., Dallas, TX 75216. Care includes skilled nursing, physical, occupational and speech therapy, homemaker services, and home health aid services. Provider must be JCAHO accredited. The following counties of Texas will be included: Callahan, Collin, Dallas, Delta, Eastland, Ellis, Franklin, Henderson, Hopkins, Hunt, Kaufman, Rains, Red River, Rockwall, Smith, Van Zandt and Wood. This contract will be for a one year period with two options to renew for a period of one year each. Issue date will be on or about June 21, 1995. Due date will be on or about July 21, 1995.
